Arterial blood supply variation in the anterior midline mandible: Significance to dental implantology

Abstract: BackgroundInter-foraminal implant placement poses a risk to the sublingual artery as it enters the mandibular midline lingual foramen. Lack of consensus on the source of this artery poses a dilemma to surgeons during management of haemorrhagic episodes. Determination of the exact source of this artery is therefore pivotal.MethodsThis was a cross-sectional descriptive study involving 34 adult human cadavers. The facial and lingual arteries were followed from the external carotid artery to determine whether they… Show more

“…Hence, if the width is not assessed erstwhile by CBCT imaging then it can lead to the perforation of the lingual cortical plate. 15 Another important anatomical landmark to be acknowledged is the presence of lingual foramen and canals. According to a study by Romanos GE et al 2012, panoramic radiograph can detect only 2.7% of lingual foramen.…”

“…The main blood supply comes from a unilateral ascending mental artery, a branch of the submental artery that emerges from the submental triangle of the neck. It is possible that the sublingual artery of the lingual artery that anastomoses with the submental artery through the mylohyoid muscle can support blood flow to the chin in cases where the submental artery is occluded . The superior labial artery does not provide any branches to the chin.…”

“…The ascending mental artery is the terminal branch of the submental artery. The submental artery can send a branch into the floor of mouth through the mylohyoid muscle thus becoming the sublingual artery . This intimate relationship between the sublingual artery and the submental artery may increase the risk of lingual infarction during chin augmentation.…”

“…A minimum bone thickness of 6 mm would be required for successful placement of a standard-sized ±4-mm-diameter implant. 2 In the present study Mandibular Body Thickness (MBT) was studied in 110 dry mandibles to evaluate the ideal length of dental implants used during the corrective procedures on the mandibular alveolar process. All the mandibles in our study had adequate bone thickness needed for implant placement without traumatizing the sublingual artery.…”
