Arthrofibrosis is the result of a T cell mediated immune response

Abstract: It is thought that an excessive fibrotic healing response with diffuse intra-articular scarring leads to arthrofibrosis after trauma and surgery around joints. To clarify the specific cellular mechanism of arthrofibrosis during arthrolysis we took fibrotic tissue samples from 18 patients at varying periods after knee trauma or surgery. Sections were stained with hematoxylin and eosin to study the overall histopathological changes. Major histocompatibility complex (MHC) class II expressing cells as well as CD3,… Show more

“…The complex functions of macrophages are enabled by the presence of different macrophage subpopulations within the joint, which fulfill distinct inflammatory and antiinflammatory roles [92]. In synovial tissue fibrosis, bone marrow derived monocytes and macrophages are found in increased numbers, which may signal their involvement in the development of synovial fibrosis [93,94]. In response to an inflammatory signal, macrophages produce TGF-β as a regulatory feedback signal to aid in the resolution of inflammation [95,96].…”

“…Technical errors that cause impingement should be distributed equally between the two groups. Development of arthrofibrosis after reconstruction has been much debated and results from an exaggerated inflammatory response, or is secondary to delayed mobilisation, infection, or sympathetic dystrophy 25 26. Patients who have a patella tendon reconstruction are susceptible to anterior knee pain and synovitis, w13 and this correlates with the development of arthrofibrosis 27.…”

“… 39 Immunohi-stological studies on arthrofibrosis have linked T-cell mediated immune response with fibrotic scar tissue formation. 40 Matthews et al . 41 also reported dense scar tissue formation on the medial side of the knee after transection of the anterior cruciate ligament in an animal model.…”
