Arthroscopic bone graft procedure combined with arthroscopic subscapularis augmentation (ASA) for recurrent anterior instability with glenoid bone defect: a cadaver study

Abstract: BackgroundGlenoid bone loss and capsular deficiency represent critical points of arthroscopic Bankart repair failures. The purpose of this study was to evaluate an all-arthroscopic bone block procedure associated with arthroscopic subscapularis augmentation (ASA) for treating gleno-humeral instability with glenoid bone loss (GBL) and anterior capsulo-labral deficiency. Our hypothesis was that these two procedures could be combined arthroscopically. The feasibility of this technique and its reproducibility, and… Show more

“…9,11 Several open and arthroscopic operative techniques are utilized to reconstruct the glenoid with a variety of autograft and allograft sources. 1 3,12 14,16,20 23,34 The Bristow-Latarjet procedure is being used with increasing frequency for addressing recurrent shoulder instability in the setting of glenoid bone loss. This surgical technique restores the congruity of the deficient glenoid by augmenting the anterior glenoid with the coracoid process.…”

“…The subscapularis has an important role in maintaining forward stability of the shoulder. Suturing part of the subscapularis tendon and joint capsule to the anterior labrum effectively enhances the anterior blocking effect of the shoulder and does not have a significant impact on rotation of the shoulder[ 13 , 14 ]. We observed a reduction in movement of the humeral head during arthroscopic subscapular augmentation.…”

“…Complications related to the Latarjet procedure have been reported in 25% of patients, in contrast to anatomical arthroscopic techniques that report low rates of complications. The process of covering the allograft during capsulolabral reconstruction may have resulted in a lower progression rate to osteoarthritis in comparison with the Latarjet procedure 19 , 23 , 27, 28, 29…”
