Objective: The purpose of this study was to compare the results of open Bankart repair versus those of modified Bristow operation in patients with recurrent anterior dislocations of the shoulder, the last of which was caused by a minor trauma. Materials and Methods: This study included 38 patients (34 (89.5%) male and 4 (10.5%) female) who presented recurrent dislocation of the shoulders with capsular laxity and who underwent an open Bankart repair or a modified Bristow operation. The mean age of the patients was 29.6 years (range, 17-60 years). The mean follow-up period was 5.5 years (range, 35 months to 9 years). A total of 25 patients (65.8%) underwent an open Bankart repair, whereas 13 (34.2%) underwent a modified Bristow operation. The treatment results were assessed using the Rowe score for instability. Results: Of the 38 shoulders assessed, 24 (63.1%) were right shoulders and 14 (36.9%) were left shoulders. Furthermore, 26 (68.4%) were the dominant shoulders of the patients, and 12 (31.6%) were the nondominant shoulders. The mean time from the first dislocation was 3.8 years (range, 10 months to 11 years). The age at which the first shoulder dislocation occurred was 20 years or younger in 7 cases (18.4%), 21-30 years in 22 cases (57.9%), 31-40 years in 6 cases (15.8%) and 41 or older in 3 cases (7.9%). The patients had experienced 4-10 recurrent dislocations in 15 cases (39.5%), 10-20 recurrent dislocations in 10 cases (26.3%) and 20 or more recurrent dislocations in 13 cases (34.2%). Gereç ve Yöntem:Tekrarlayan travmatik anterior omuz çıkıklı ve kapsüler laksisitesi olan, açık Bankart veya modifiye Bristow ameliyatları ile tedavi edilen 38 (dağılım, 34 (%89.5) erkek, 4 (%10.5) kadın) hasta çalışmaya alındı. Ortalama yaşları 29.6 yıl (dağılım, 17-60 yıl). Ortalama takip süresi 5.5 yıldı (dağılım, 35 ay-9 yıl). Hastaların 25'ine (%65.8) açık Bankart tamiri, 13'üne (%34.2) modifiye Bristow ameliyatı yapıldı. Tedavi sonuçları Rowe'un skorlama sistemine göre yapıldı. Açık Bankart tamiri yapılan hastalarda Rowe puanı 85.6, modifiye Bristow ameliyatında 81.9 bulundu. Rowe değerlendirme kriterlerine göre açık Bankart ve modifiye Bristow ameliyatlarının çok iyi ve iyi toplamları arasında istatistiksel olarak anlamlı bir fark bulunmadı (p>0.05).Sonuç: Açık Bankart tamiri ve modifiye Bristow ameliyatları için uygun hasta seçilmesi çok önemlidir. Uygun hasta seçimi yapıldığı takdirde bu iki ameliyat tekniği kapsüler laksisiteli hastaların tedavisinde başarılı bir tedavi yöntemidir.

zeigt nach einer traumatischen Erstluxation und v. a. bei chronisch rezidivierender Instabilität strukturelle Schäden sowohl am Kapsel-LabrumKomplex (z. B. Bankart-Läsion), als auch an den knöchernen Gelenkpartnern (z. B. Hill-Sachs-Defekt, [1]). In der Literatur wird von einer Präva-lenz des knöchernen Defekts am Glenoid zwischen 8 und 73 % berichtet [2-4]. Die Bedeutung knöcherner Läsionen am Glenoid wurde in klinischen und biomechanischen Untersuchungen nachgewiesen [5, 6]. Eine Defektgröße von etwa 21 % der Glenoidoberfläche führt zu einer signifikanten Reduktion der glenohumeralen Stabilität und kann nicht mehr ausreichend durch eine alleinige Rekonstruktion von Weichteilen kompensiert werden [7]. Burkhart u. Beer [8] konnten zeigen, dass durch die arthroskopische Bankart-Operation bei instabilem Schultergelenk mit zusätzlichem Glenoiddefekt keine ausreichende Stabilität erzielt werden kann. Die Autoren berichten von einer Reluxationsrate von bis zu 89 % bei Wiederaufnahme sportlicher Aktivitäten. Zur Behandlung der chronischen Instabilität mit knöchernem Glenoiddefekt stehen mehrere Techniken zur Verfügung: Das Verfahren nach Eden-Hybinette, die knöcherne Augmentation nach Bristow-Latarjet und die trikortikale Beckenkammspanplastik und deren Modifikationen sind die wesentlichen anatomischen und extraanatomischen Varianten zur Versorgung dieser Defekte. Bei allen wird Fremdmaterial verwendet, was zu teils erheblichen Komplikationsraten führen kann [9, 10]. Die J-Span-Augmentation wurde 1988 erstmalig angewandt und verfolgt als einzige das Ziel einer implantatfreien anatomischen Rekonstruktion der ursprüng-lichen Glenoidmorphologie [11]. Langzeitergebnisse nach offener J-Span-Plastik zeigen in > 95 % sehr gute funktionelle Resultate [11]. Diese ursprünglich offene Methode wurde zu einem arthroskopische Verfahren weiterentwickelt [12]. Im Folgenden werden die Kurzzeitergebnisse nach arthroskopischer Technik anhand einer prospektiven Studie an 10 Patienten vorgestellt. Studiendesign und UntersuchungsmethodenIn die vorliegende prospektive Studie wurden 10 konsekutiv aufgenommene Patienten mit traumatischer, rezidivierender ventraler Schulterinstabilität mit knöcher-nem Glenoiddefekt inkludiert. Die Indikation zur operativen Versorgung wurde nach definierten Kriterien gestellt: chronische Instabilität des betroffenen Schultergelenks mit einem Knochendefekt von > 15 % oder von bereits > 10 % im Falle einer vorausgegangenen Operation.
