Arthroscopic Treatment of Shoulder Instability: A Systematic Review of Capsular Plication Versus Thermal Capsulorrhaphy

Abstract: Objective: Shoulder instability is a common disorder of the shoulder that can result in debilitating pain and decreased function. Poorly treated cases of instability result in excessive mobility, possibly leading to labral tears and degenerative arthritis. The purpose of my systematic review was to compare the effectiveness of 2 popular arthroscopic techniques used to reduce shoulder instability: capsular plication and thermal capsulorrhaphy.Data Sources: Articles were retrieved from PubMed, Cochrane Library, … Show more

“…These procedures had high failure rates due to rebound ligament stretching. 4 Arthroscopic capsular shift procedures that use capsular plication have also been developed to mimic the gold-standard open procedure. Techniques vary, with the use of different suture or tack applications, plication and pleating variations, humeral-or glenoid-based shifts, direction of approach, and posterior pinch-tuck methods.…”

“…2,3 Surgical options to reduce the capsular volume include open capsular shift and plication, arthroscopically capsular plication, and thermal capsulorrhaphy. 2,4,5 The open anteroinferior capsular shift-plication involves incising, overlapping, and suturing the capsule to reduce redundancy. Arthroscopic techniques include the thermal capsulorrhaphy and the arthroscopic version of the open capsular shift-plication procedure.…”

“…Arthroscopic procedures are typically preferred due to their minimally invasive and conservative nature. 12 Arthroscopic Bankart repair (ie, anatomic repair of Bankart lesion) is commonly used 13 and aims to restore the labrum and re-establish appropriate tension in the joint capsule and ligaments. 14 In certain cases, open (ie, surgery via a large incision) procedures are preferred.…”
