Objective This study investigates therapists' perceptions and experiences using Generative AI in couple and family therapy, focusing on its benefits, challenges, and ethical considerations. Background As Generative AI continues to transform various industries, its application in therapeutic practices, particularly couple and family therapy (CFT), remains underexplored. CFT introduces unique dynamics and complexities, which pose specific challenges for AI integration in therapy settings. Method This qualitative study utilized a purposive sample of 25 experienced couple and family therapists. In-depth, semi-structured online interviews were conducted, and constructivist grounded theory was applied to analyze key themes emerging from the data. Results Three central themes were identified: Ethical and Privacy Concerns, the Impact of AI on the Therapeutic Relationship, and Future Directions and Worries. These themes offer a detailed understanding of AI's potential to enhance therapy while highlighting the significant risks and ethical dilemmas it introduces. Conclusion This study reveals that while AI can augment therapeutic practices through data-driven insights and improve intervention effectiveness, there are concerns about data confidentiality and the depersonalization of therapy. The findings underscore the need for a balanced approach to AI integration in therapy, ensuring the maintenance of the human connection central to therapeutic success. Implications: The study emphasizes the importance of developing ethical guidelines for AI use in therapy, enhancing technological literacy among therapists, and ensuring informed client consent. Future research should explore the long-term impact of AI on therapeutic relationships and address privacy concerns, providing strategies for the ethical integration of AI in couple and family therapy.
