Aspirin Hypersensitivity in Patients with Chronic Rhinosinusitis and Nasal Polyposis: Frequency and Contributing Factors

Abstract: ASA hypersensitivity is common in patients with CRSwNP in Tehran, Iran. Patients at risk for AERD should be evaluated for the presence of ASA hypersensitivity with ASA provocation challenge test to confirm the diagnosis.

“…We further compared the prevalence of asthma, allergies, and ASA syndrome (aspirin sensitivity, asthma and CRSwNP), the more commonly coexisting comorbidities in CRSwNP, among TAS2R38 genotypes in 84 patients. Chi‐square analysis showed no statistically significant difference between PAV taster and AVI nontaster, suggesting no association between receptor functionality and CRS‐related comorbidities (χ 2 : 6.113; P = 0.295).…”

“…Aspirin‐exacerbated respiratory disease (AERD) represents a challenging syndrome of adult‐onset asthma, chronic rhinosinusitis with nasal polyposis (CRSwNP), and non–immunoglobulin E (IgE)‐mediated hypersensitivity to cyclooxygenase‐1 (COX‐1) inhibitors, such as acetylsalicylic acid (ASA) and other nonsteroidal anti‐inflammatory drugs (NSAIDS) . First recognized by Widal et al .…”

“…Among patients with CRSwNP, prevalence of AERD has been reported at 8% to 26%, with up to 40% developing aspirin sensitivity during the course of their disease . Although aspirin challenge represents the gold standard for diagnosis of AERD, understanding of the pathophysiology contributing to AERD remains incomplete.…”

“…Patients with NERD present as moderate‐to‐severe asthma, in which eosinophils are activated in the upper and lower airway mucosa . Compared to aspirin‐tolerant asthma (ATA), NERD is less associated with atopy; however, atopy is often present in patients with NERD .…”
