Abstract

Bilateral perirenal haematoma: case reportAn approximately 61-year-old woman developed bilateral perirenal haematoma during prophylactic treatment with aspirin for ischaemic stroke.The woman was diagnosed amyloid light-chain amyloidosis at the age of 53 years. She had undergone autologous stem cell transplantation, and had received bortezomib, lenalidomide and dexamethasone. However, her renal function gradually worsened. Her medical history was significant for mild hypertension. Subsequently, she had persistent severe proteinuria secondary to amyloid light-chain amyloidosis that resulted into a chronic kidney disease. She was started on peritoneal dialysis due to end stage renal disease after 8 years of the amyloidosis diagnosis. She had a history of ischaemic stroke, due to which she had been receiving lowdose aspirin as a propylaxis [dosage and route not stated] along with other co-medications. Six months after peritoneal dialysis initiation (at 61 years of age), a contrast-enhanced CT scan of left kidney showed perirenal haematoma. Her blood tests were also significant for severe anaemia (Hb decreased to 7.3 mg/dL).The woman received conservative treatment and her symptoms improved. Her peritoneal dialysis was continued. However, one year later, she developed second perirenal haematoma on other kidney along with haemoperitoneum on the contralateral side. Thereafter, she was hospitalised for blood transfusion due to hypotension. Her treatment with aspirin was stopped. Her residual renal function deteriorated due to severe anaemia (6.9 mg/dL), and peritoneal dialysis was stopped. She underwent series of laboratory tests that confirmed occurrence of perirenal haematoma. The bilateral perirenal haematoma was attributed to her aspirin use [duration of treatment to reaction onset and outcome not stated].
