Assessing and Treating Patients With Neuropathic Pain

Abstract: Neuropathic pain (NP) is a significant source of suffering, disability, and impairment, as well as an enormous cost to society. Historically, pharmacologic treatment has been limited to drugs approved for other conditions, including anticonvulsants, antidepressants, antiarrhythmics, and opioids, but in the past 2 decades several drugs have been approved by the Food and Drug Administration specifically for NP. Understanding the underlying pathophysiology and clinical presentation of the various causes of NP sta… Show more

“…Chronic neuropathic and inflammatory pain have been estimated to affect one-sixth of the population and impair their quality of life. , Neuropathic pain is a devastating consequence of injury or disease that affects the somatosensory system, either centrally or peripherally . Chronic inflammation can lead to hypersensitization of neurons and the release of inflammatory mediators, such as bradykinins and prostaglandins, thereby promoting the process of inflammatory pain .…”

“…[1][2][3] Few clinical guidelines for the treatment of sciatica exist, and evidence regarding effective medical treatments is limited. 2,3 Treatment with pregabalin (Lyrica, Pfizer) has been shown to be effective in reducing some types of neuropathic pain, including postherpetic neuralgia and diabetic peripheral neuropathy, 4,5 as well as allodynia and hyperalgesia from several conditions, [6][7][8] and some guidelines recommend pregabalin for the treatment of pain with neuropathic features. 5 Pregabalin therefore represents a potential treatment for sciatica.…”
