Assessing fidelity of cognitive behavioral therapy in rural VA clinics: design of a randomized implementation effectiveness (hybrid type III) trial

Abstract: BackgroundBroadly disseminating and implementing evidence-based psychotherapies with high fidelity, particularly cognitive behavioral therapy (CBT), has proved challenging for many health-care systems, including the Department of Veterans Affairs, especially in primary care settings such as small or remote clinics. A computer-based tool (based on the coordinated anxiety learning and management (CALM) program) was designed to support primary care-based mental health providers in delivering CBT. The objectives o… Show more

“…Our findings suggest that the likelihood of clinical uptake after training is also heavily influenced by implementation climate; leadership at call centers wishing to implement SPI will need to address center constraints that conflict with successful utilization of SPI (e.g., short call times, emphasis on current crisis). While studies examining both implementation and effectiveness during dissemination trials are becoming more common in other disciplines (Cucciare et al., ; Damschroder et al., ; Zatzick et al., ), this approach has only begun to be applied to suicide prevention (Stanley, ; Labouliere et al., ). Future studies should further investigate what differentiates settings that are and are not conducive to SPI, and how these obstacles to effective implementation can best be overcome.…”

“…39 Using the frameworks and study designs of implementation science, researchers have discovered the key barriers and facilitators to clinician and organizational use of CBT treatments, as well as the strategies critical to its implementation across diverse settings. [40][41][42][43][44] This success story can exist within obstetrics.…”
