Human Papillomavirus and Head and Neck Cancer: Psychosocial Impact in Patients and Knowledge of the Link – A Systematic Review

Head and neck cancer (HNC) currently affects approximately 11 200 people in the UK, with an increasing proportion known to be caused by the human papillomavirus (HPV). We undertook a systematic review of studies measuring the psychosocial impact of HPV-related HNC and also studies measuring knowledge about the link between HPV and HNC among different populations. Searches were conducted on MEDLINE, Embase, PsycINFO, CINAHL Plus and Web of Science, with reference and forward citation searches also carried out on included studies. Studies were selected if they (i) were original peer-reviewed research (qualitative or quantitative), (ii) mentioned HPV and HNC, (iii) measured an aspect of the psychosocial impact of the diagnosis of HPV-related HNC as the dependent variable and/or (iv) measured knowledge of the association between HPV and HNC. In total, 51 papers met the inclusion criteria; 10 measuring psychosocial aspects and 41 measuring knowledge of the link between HPV and HNC. Quality of life in those with HPV-positive HNC was found to be higher, lower or equivalent to those with HPV-negative HNC. Longitudinal studies found quality of life in patients was at its lowest 2–3 months after diagnosis and some studies found quality of life almost returned to baseline levels after 12 months. Knowledge of the link between HPV and HNC was measured among different populations, with the lowest knowledge in the general population and highest in medical and dental professionals. Due to the limited studies carried out with patients measuring the psychosocial impact of a diagnosis of HPV-positive HNC, future work is needed with the partners of HPV-positive HNC patients and health professionals caring for these patients. The limited knowledge of the association between HPV and HNC among the general population also indicates the need for research to explore the information that these populations are receiving.

Introduction: This article describes the knowledge, attitudes and practices of users of the School of Dentistry at the Universidad de Antioquia about oral cancer.Materials and methods: Study with a quantitative, descriptive approach; a collection instrument was designed with which sociodemographic variables were ascertained, and the knowledge, attitudes and practices of users of the School of Dentistry at the Universidad de Antioquia were evaluated.Results and discussion: Although most of the people surveyed stated that they are aware of the existence of oral cancer, they do not recognize the warning signs of oral cancer and how they can prevent it; these results do not vary in individuals who are taken care of at the School. Despite these results, most respondents examine themselves intra-orally, although they do not have clear instructions on how to do it. Conclusions:The fact that people ignore how to act against oral cancer despite recognizing its existence is largely related to health professionals since, although the majority of respondents were attended to at the School of Dentistry and even in other healthcare centers, their knowledge, attitudes and practices did not vary much from those of people who did not have access to these programs. Therefore, it is suggested that an educational strategy be implemented to increase the knowledge of detection and prevention of oral cancer, in which the dentist is the main actor. Câncer bucal: conhecimentos, atitudes e práticas dos usuários da Faculdade de Odontologia da Universidad de Antioquia, Colômbia, 2016Resumo Introdução: este artigo descreve os conhecimentos, atitudes e práticas dos usuários da Faculdade de Odontologia da Universidad de Antioquia sobre o câncer bucal.Metodologia: estudo com abordagem quantitativa, descritiva; elaborou-se um instrumento de coleta de dados com o qual se questionou por variáveis sociodemográficas e se avaliaram os conhecimentos, atitudes e práticas dos referidos usuários.Resultados e discussão: embora a maioria das pessoas pesquisadas expressa saber da presença do câncer de boca, não reconhecem quais são os sinais de alarme dele e como podem preveni-lo; esses resultados não variam em pessoas que são atendidas em tal faculdade. Apesar dos resultados, grande parte dos participantes avalia-se intrabucalmente, mesmo não tendo instruções claras de como fazer isso.Conclusões: o fato de que as pessoas desconheçam como devem agir diante do câncer de boca, mesmo reconhecendo sua existência, está relacionado em grande medida com os profissionais da saúde, já que a maioria dos participantes recebia atendimento na Faculdade de Odontologia e inclusive em outros centros de saúde e, ainda assim, seus conhecimentos, atitudes e práticas não variavam com aqueles das pessoas que não tinham acesso a esses programas. Portanto, sugere-se implantar uma estratégia educativa que possa aumentar o conhecimento de detecção e prevenção de câncer de boca na qual o odontologista seja o ator principal.

ResumoObjetivo: Avaliar o conhecimento de estudantes de diferentes cursos da área da saúde da Universidade Vale do Rio Verde de Três Corações (Minas Gerais, Brasil) sobre o câncer de boca. Método: Foi realizado um estudo observacional, no qual 314 estudantes dos cursos de farmácia, enfermagem, nutrição e odontologia responderam a um questionário estruturado com 17 perguntas divididos entre autoavaliação e caraterísticas objetivas sobre o câncer de boca. Resultados: Uma parcela representativa da população universitária sabe que o câncer pode acometer a boca e que o tabagismo e etilismo são fatores de risco, entretanto, mostraram desconhecimento sobre a faixa etária mais afetada. Notou-se que os 4 cursos apresentaram nível de conhecimento deficiente, contrapondo-se ao questionário de auto avaliação. Houve uma tendência de alunos do primeiro e segundo anos da graduação obterem pontuação maior. Conclusão: Os dados obtidos sugerem a necessidade de estratégias direcionadas ao processo ensino/aprendizagem, visando o melhor aproveitamento e fixação do conhecimento acerca do câncer de boca, bem como o enfoque no desenvolvimento de políticas de prevenção desta doença no projeto pedagógico destes cursos. AbstractIntroduction: To evaluate the knowledge of students from different health courses at the Vale do Rio Verde University of Três Corações (Minas Gerais, Brazil) on oral cancer. Methods: An observational study was conducted in which 314 students from the pharmacy, nursing, nutrition and dentistry courses answered a structured questionnaire composed of 17 questions. The questions were divided in two parts: self-evaluation and objective characteristics about oral cancer. Results: A representative sample of the students answered that cancer can affect oral tissues and showed mastery over the role of cigarette as a risk factor, however, they were deficient in demonstrating the most affected age group. All groups presented critical knowledge levels, opposed to selfevaluation questions. There was a tendency of first and second course year to settle more questions. Conclusion: The data obtained suggest the need for strategies directed to the teaching/learning process, aiming the best use and establishment of knowledge about oral cancer, as well as the focus on the development of prevention policies for oral cancer in the pedagogical project of the evaluated courses.
