Assessment following Hand Trauma: A Review of some Commonly Employed Methods

Abstract: Accurate assessment strategies underpin appropriate implementation and review of surgical and therapeutic intervention. This paper explores some of the commonly described assessment methods used following hand injury, including measures of range of motion (ROM), strength, sensation, hand function and patient self-evaluation. The strengths and limitations of some methods are discussed. It is suggested that accurate assessment tools and standardised approaches to assessment will promote effective evaluation of i… Show more

“…This has subsequently been further explored and concerns remain regarding patient relevance and the sensitivity and specificity of some measures (Bücher et al 2003). A literature review (Bücher & Hume 2002) suggests variable quality of assessment tools and anecdotally, some practitioners report inconsistency between recommended assessment guidelines and practice -suggestions that are confirmed in part in the literature (Jerosch-Herold 2002, Pratt & Burr 2001, Marcuzzi et al 1998. It was hypothesised that factors influencing practice might include limitations of time, resources, expertise and training opportunities.…”

“…Although hand impairments can cause activity limitations, this relationship is rather complex (9)(10)(11). It is, thus, important to assess not only body functions (impairments), but also a person's (limitations to perform) activities in order to determine how different hand conditions impact the daily lives of individual patients (6,8,(12)(13)(14)(15)(16)(17)(18)(19).…”

“…This, however, requires consensus on which of the available validated instruments, that are also reliable and responsive, should be used to assess (and preferably predict) a patient's functioning and/or to evaluate outcomes of different interventions. Many tools are available to assess impairments and activity limitations in patients with hand conditions, but there is currently no standardized accepted core set for use with this patient group (6,12,14,18,20). To support the development of such a core set, we first conducted 2 systematic reviews in order to identify 23 instruments that can be used to evaluate activity limitations in patients with hand conditions (18,19).…”

“…fl exor tendon injury has the potential to compromise function, create disability and impact on a patient's well being, as declared explicitly by Bucher and Hume (2003). The process of tendon repair and subsequent rehabilitation is a traumatic and potentially devastating experience.…”
