Registered nurses (RNs) with primary care experience are in short supply, and the employment demand for RNs is projected to grow 15% from 2016 to 2026. 1 Increased demand will occur for several reasons, including an increased emphasis on preventive care; growing rates of chronic conditions, such as diabetes and obesity; and demand for health care services from persons born between 1946 and 1964, as they live longer and lead more active lives. 2 Careers in population health and primary care-based nursing will be among the greatest nursing demand, but only about 10% of nurses choose careers in primary care. 3 New graduate nurses are generally not recruited to work in the primary care settings and are often encouraged to gain practice experience before working in the primary care setting. 3 These current career trends and workforce perceptions indicate an inadequate number of nurses prepared for roles in primary care settings to meet the growing demands. Primary care and ambulatory care are not traditional settings for clinical experiences. Therefore, exposure of prelicensure nursing students to population health concepts in primary care settings is a priority.To address this issue, a nationally funded initiative was implemented to increase knowledge of and exposure to population health and careers in the primary care setting among undergraduate nursing students. BackgroundMedicare annual wellness visits (AWVs) are prevention‐focused healthcare visits free to Medicare recipients. These visits focus on health maintenance, health risk assessment, prevention of illness, and maintaining independence, all of which are within the scope of registered nurse (RN) practice as well as aligned with what matters, medication, mentation, and mobility — the 4Ms — of age‐friendly health care. The objective of this pilot study was to evaluate the implementation of the 4Ms in the context of RN‐led Medicare AWVs in a primary care practice.MethodsIn a primary care practice with approximately 2500 patients, including approximately 571 of whom were enrolled in Medicare, RN‐led Medicare AWVs were implemented, incorporating the 4Ms framework. During this time, data were collected on the effect of the AWV on access to care—conceptualized here as the number of visits available as well as the type of clinician open to staff these visits. Data collection also included patient responses to the 4Ms question “what matters most?”ResultsOverall, the RN‐led visits were successful and beneficial to the practice. Each RN‐led visit allowed for 2 additional acute or monitoring visits per provider (nurse practitioner, MD) per day, increasing patient access to their primary care providers. Inclusion of the 4Ms questions facilitated discussion around overall mental and emotional well‐being, life stressors, quality of life, and goals of care.ConclusionRN‐led Medicare AWVs incorporating the 4Ms framework enhances the role of RNs in primary care by focusing on a health promotion role, utilizing RNs to their full scope of practice. RN‐led AWVs increase provider availability for acute and chronic care appointments, as well as foster conversations around quality of life, as well as mental and emotional well‐being.

Objetivo: Descrever os caminhos percorridos pelos futuros enfermeiros na busca por complementar a formação em saúde mental. Métodos: Estudo descritivo-exploratório, com abordagem qualitativa, realizado de setembro de 2020 a maio de 2021, com 36 estudantes, por meio de entrevista semiestruturada, utilizando como recurso tecnológico o aplicativo WhatsApp®. Os achados foram analisados à luz da técnica de Análise de Conteúdo de Bardin e discutidos com a literatura científica sobre a temática. Resultados: A necessidade de complementação e aperfeiçoamento na formação em saúde mental acontece devido a disciplina ter pouca carga horária, conteúdos superficiais e poucas atividades práticas nos serviços de saúde mental. A busca também se dá pela afinidade com a área e o desejo de especializar para trabalhar no campo da saúde mental. As estratégias mais utilizadas são ligas acadêmicas, projetos de extensão, iniciação científica e participação em grupos de estudos. Considerações Finais: Embora configurem espaços de protagonismo para os estudantes, a busca por atividades complementares revela a fragilidade da formação nas disciplinas e que essa estratégia é escolhida apenas pelos estudantes que possuem afinidade com o campo de atuação.
