Assessment of tuberculosis infection during treatment with biologic agents in a BCG-vaccinated pediatric population

Abstract: Biologic therapies, such as tumor necrosis factor-alpha (TNF-α) blockers, are commonly used to treat rheumatological diseases in childhood. Screening patients for tuberculosis (TB) is highly recommended before starting therapy with TNF-α blockers. Despite appropriate screening, TB still remains a problem in patients receiving anti-TNF therapy in countries where TB is not endemic. TB in anti-TNF-treated patients is often diagnosed late due to altered presentation, and this delay results in high morbidity and mo… Show more

“…3,5,8,14,16,17,20,26,31,37 There were only eight specific papers on TB in children with rheumatic diseases using biologics. One was a case-control study 12 that evaluated the risk of TB in children with JIA in Taiwan, two were retrospective studies 4,35 that evaluated the safety of anti-TNFα/ biologics in relation to TB, one was a prospective study 10 that evaluated TB during treatment with biologic agents in a population vaccinated with bacillus Calmette-Guérin (BCG), and four were case reports. 15,22,24,33 Screening for TB before starting biologic therapy was performed in just over half (19/37) of the studies.…”

[3][4][5][8][9][10][11]14,15,19,21,24,27,28,30,[33][34][35][36] Of the 30 cases of TB disease (PTB: 26, EPTB: 4), 8/30 occurred in patients who had been screened before biologic therapy and 22/30 in those not screened. Of the eight specific studies on TB, screening was conducted in six; of the two studies in which there was no report on screening for TB, one is the case that progressed to death.…”

“…Of the limited studies published in pediatric population, in a study conducted in Turkey, 71 children with inflammatory diseases were screened for LTBI prior to anti-TNF treatment and 8.45% of the children were diagnosed with LTBI with TST cut-off value ≥15 mm and/or QFT-GIT positivity. 24 In our study, we used current local guidelines which suggest TST cut-off value ≥5 mm and/or T-SPOT.TB positivity and diagnosed 18 (31.5%) of our patients with LTBI. Difference between LTBI rates is obviously the result of different TST cut-off values.…”

“…Moreover, only four departments were selected to meet the purpose of the study: food and beverages, frontline office, housekeeping, and marketing departments [56]. Furthermore, purposive sampling was chosen to focus on people with particular characteristics compatible with the research purpose [81]. Four hundred and nine questionnaires were returned out of 700 distributed among the participants.…”
