Assessment of work instability in spondyloarthritis: a cross-sectional study using the ankylosing spondylitis work instability scale

Abstract: Work instability was found to be high and its main determinant was patient's global assessment. The predictive validity of the AS-WIS in terms of job retention should be further assessed.

“…Compared with joint involvement in rheumatoid arthritis (RA), inflammation in SpA mainly takes the form of enthesitis and commonly results in bone formation and ankylosis rather than bone erosion (Marzo-Ortega et al 2002 ; McGonagle et al 2014 ). Nevertheless, the consequences of these changes also lead to functional limitations, poor quality of life, loss of working hours, and even decreased life expectancy (Braun and Pincus 2002 ; Fabreguet et al 2012 ). Prognosis is often poorer because of the delay in diagnosis, especially in patients with isolated axial involvement (Rudwaleit et al 2005 ).…”

“…Involvement of peripheral joints, of hips or of the cervical spine, has a significant effect on functional capacity. In other studies, hip-involvement or impairment of specific indexes for AS were also highly associated with work-disability [7,[15][16][17].…”

“…Furthermore, the physical limitations of axSpA can affect basic activities such as eating, washing, dressing, and social activities, as well as the ability to work. Cross‐sectional studies have reported that 10.5%‐32% of patients were forced to give up their jobs due to the disease, with limitations to work ability described by about half of all respondents . Taken together, the evidence shows that axSpA patients face significant challenges, and the main goal of treatment should therefore be to restore and preserve quality of life and normal function for patients as much as possible.…”
