We developed and evaluated a rapid and simple multiplex microsphere assay for the quantification of specific IgG and IgA antibodies against meningococcal serogroup A, C, W, and Y capsular polysaccharides in serum and saliva. Meningococcal polysaccharides were conjugated to distinct magnetic carboxylated microspheres, and the performance of the assay was assessed using the CDC1992 standard meningococcal reference serum and a panel of serum and saliva samples. The standard curve was linear over an eight 3-fold dilution range in the IgG assay and a seven 3-fold dilution range in the IgA assay. No cross-reactivity was discovered, and the assay showed high specificity with >91% homologous inhibition and <11% heterologous inhibition for all serogroups and immunoglobulin classes. Lower limits of detections were <280 pg/ml for IgG and <920 pg/ml for IgA antibodies. The assay was reproducible, with a mean coefficient of variation of <5% for intra-assay duplicates, a mean coefficient of variation of <20% for interassay repeated analysis with different conjugations of microspheres, and a mean coefficient of variation within 25.8% for interoperator variation. The assay showed good correlation to the standard meningococcal polysaccharide enzyme-linked immunosorbent assay (ELISA) for detection of serum antibodies. This multiplex assay is robust and reliable and requires less sample volume, and less time and workload are needed than for ELISA, making this method highly relevant for serological and salivary investigations on the effect of meningococcal vaccines and for immunosurveillance studies. Meningococcal disease continues to be a significant public health problem, although vaccines used in national immunization programs or mass vaccination campaigns have reduced the incidence of the disease in several countries (1). The capsular polysaccharide is an important antigen and virulence factor (2), and the most widely used meningococcal vaccines are based on these polysaccharides. Such vaccines have been shown to be effective for serogroups A, C, W, and Y, four of the five major diseasecausing meningococcal serogroups (1, 3), and have been available and widely used for nearly half a century.To evaluate the effect of meningococcal vaccines and determine protection against disease, serogroup-specific serological measures are used. Serum bactericidal activity (SBA) has become the most widely used surrogate of protection and is the basis for licensure of the recent meningococcal vaccines (4). However, this method is highly time consuming and requires specialized laboratories and highly standardized biological reagents. Quantitation of specific anti-meningococcal polysaccharide antibodies, on the other hand, is more suitable for large immunosurveillance studies and contributes to a broader understanding of the immune response. A tetravalent meningococcal conjugate vaccine for serogroups A, C, W, and Y (MenACWY) is one of several recommended routine vaccinations for all new U.S. military recruits during enlisted basic training or officer accession training. Military recruits at all basic training centers routinely receive five or more concomitant vaccinations, including combination vaccines, within the first week of arrival at training facilities. While the safety and immunogenicity of coadministered vaccines in infants and children have been widely studied (1-9), the same approach has not been systematically followed for immunizations in adults and, in particular, for meningococcal polysaccharide conjugate vaccines in military personnel.The effects on immunogenicity of concomitant vaccine administration are variable and may be vaccine specific. For example, inactivated polio vaccine (IPV) given alone or coadministered with other vaccines in military personnel showed that seroconversions to polio were more likely when multiple vaccines were administered (10). In contrast, it has been reported that immune response to the concomitant administration of the tetanus-diphtheria-acellular pertussis (Tdap) vaccine and the trivalent influenza vaccine was reported to be noninferior to sequential administration despite lower concentrations of antibody to tetanus, diphtheria, and pertussis toxin components (11, 12). West et al. reported that antibodies to rubella virus and pertussis filamentous hemagglutinin were below predicted levels in healthy adults who received concurrent administration of the bivalent Haemophilus influenzae type b (Hib)-hepatitis B vaccine (HBV) with priming doses of the diphtheria, tetanus, acellular pertussis (DTaP) vaccine and a booster dose of DTaP, oral polio vaccine, IPV, or measles, mumps, and rubella (MMR) vaccine (13). Bar-On et al. carried out a meta-analysis of 18 studies comparing the immune responses to and safety of concomitant and sequential administration of . The capsular polysaccharide of Neisseria meningitidis serogroup W135 is expressed in both O-acetyl-positive (OA ؉ ) and O-acetyl-negative (OA ؊ ) forms. This study investigates the impact of OA status (OA ؉ versus OA ؊ ) on serological measurements of anti-W135 immunoglobulin G (IgG) antibodies in immunized adults. W135-specific serum antibody assignments were made for 28 postimmunization sera from adults by enzyme-linked immunosorbent assay using the meningococcal standard reference serum CDC1992. The established IgG concentration in micrograms per milliliter ([IgG]g/ml) for CDC1992 against OA ؉ antigen (16.2 g/ml) was used as a reference to assign a concentration of 10.13 g/ml IgG against OA ؊ antigen by cross-standardization. Overall, the IgG assignments for these sera were higher against OA ؉ antigen (geometric mean concentration [GMC] ‫؍‬ 7.16 g/ml) than against OA ؊ antigen (GMC ‫؍‬ 2.84 g/ml). However, seven sera showed higher specific [IgG]g/ml values against the OA ؉ antigen than against the OA ؊ antigen. These sera were also distinguished by the inability of fluid-phase OA ؊ antigen to compete for antibody binding to OA ؉ solid-phase antigen. Although there was no overall difference in functional activity measured by complement-mediated serum bactericidal assay (SBA) against OA ؉ and OA ؊ target bacteria (geometric mean titers of 9,642 and 9,045, respectively), three serum specimens showed a large difference in SBA antibody titers against OA ؉ versus OA ؊ W135 target bacteria, which may reflect different epitope specificities for these sera. Our data indicate that, for some sera, the agreement in anti-OA ؉ versus anti-OA ؊ W135 IgG assignments is serum specific and does not reflect the functional (killing) activity in vitro.
