OBJECTIVE:To identify predictive indicators of institutionalization of the elderly. METHODS:A case-control study was carried out with 991 elderly individuals in the city of Pelotas, (Southern Brazil), from 2007 to 2008. The cases of institutionalized elderly adults (n = 393) were detected using a census of all long-stay institutions for the elderly in the city. The population controls (n = 598) were randomly selected using a comprehensive health survey. Pearson's chi-square test and linear trends were used to compare groups in the crude analysis; and the binary logistic regression model of the adjusted analysis, with the effects expressed as odds ratios. RESULTS:Institutionalization was more frequent in females (OR = 1.96, 95%CI 1.31, 2.95). Elderly with advanced age (OR = 3.23 and OR = 9.56 for age groups 70-79 and ≥ 80 years, respectively), those who lived without a partner (single, divorced or widowed), and those who had no formal schooling or had a functional disability preventing them from performing basic activities for daily living were more likely to be institutionalized. An inverse trend between the incidence of elderly institutionalization and the level of physical activity was observed, where somewhat active and inactive subjects were more likely to be institutionalized (OR = 1.71 and OR = 4.73, respectively). CONCLUSIONS:Of the factors examined, age ≥ 80 years, living without a partner and being physically inactive were the indicators most strongly associated with institutionalization. The encouragement of informal care through cultural and educational activities focused on the role of the family in caring for the elderly can prevent the institutionalization of these individuals. This text has three basic objectives: First it focuses on the processes of policy-making and institutionbuilding in Primary Health Care in Brazil and Canada. The second objective is a preliminary appraisal of the basic, common, and differentiating points between the two countries with regard to such processes; finally, the third objective seeks to briefly discuss the role of Brazilian and Canadian physical therapists in health care teams, comprised by allied health professionals. The first part of this study highlights PHC in Brazil, with a particular interest in the ways and means adopted by recent health assistance models that emphasized, or believed in, the role of the physical therapist. The next section deals with PHC in Canada, in a summarized way. The recent Brazilian and Canadian literature on this central topic was important for this text; in addition official documents and discussion papers were used. In conclusion, the authors claim that PHC in both countries, including many other regions of the developed and developing world, require the integration of services into a systemic (though not asphyxiating) matrix. The authors also stress the need for health promotion nationwide, and, in particular, a special attention to the role of physical therapists in the operation of health care teams in family health and PHC. ResumoO texto aborda os processos de constituição e institucionalização da Atenção Primária à Saúde no Brasil e no Canadá; os pontos de interseção e de distanciamento entre os dois países; e a inserção profissional dos fisioterapeutas brasileiros e canadenses nas equipes multidisciplinares. A primeira parte deste estudo focaliza a atenção primária no Brasil, os caminhos e estratégias de reorientação do modelo assistencial e o papel que coube nesse modelo, de modo crescente, ao fisioterapeuta. Após situar o contexto nacional, o tópico seguinte trata da atenção primária à saúde no Canadá e da atuação deste profissional nas equipes primárias de saúde. No exercício absolutamente preliminar de contrastar as duas experiências nacionais, a brasileira e a canadense, foi utilizada a literatura nacional e estrangeira sobre a temática central, além de documentos e discussion papers. O texto procurou chamar a atenção para a necessidade e importância da participação de fisioterapeutas nas equipes multiprofissionais e as diferenças na atuação deste profissional tanto no Brasil quanto no Canadá. Desnecessário acrescentar que os autores intentaram não uma "análise" comparativa, mas, sim, uma breve exposição sobre constrastes e semelhanças nos rumos da atenção primária, ao longo da história mais recente desses dois países.

RESUMO O objetivo do estudo aqui relatado foi de estimar o grau de implantação da Atenção Domiciliar Tipo 1 (AD1). Para tanto, optou-se por realizar entrevistas semiestruturadas, observação das práticas e análise documental, em estudo de caso único no município de Camaçari (BA). Os resultados indicaram grau intermediário de implantação. A atenção ao usuário foi o componente de nível mais avançado, apesar da baixa pontuação das equipes em relação à promoção da saúde e à cogestão do plano de cuidados. Fragilidades na Atenção à Família e Cuidadores e Gestão da Atenção Domiciliar ficaram evidenciadas. Recomenda-se reorganização de aspectos do processo de trabalho das equipes e da gestão municipal da atenção básica mediante investimento no apoio institucional. PALAVRAS-CHAVE
