Association between daily haemodialysis, access to renal transplantation and patients' survival in France

Abstract: Our study showed that in France, the likelihood of undergoing renal transplantation after being waitlisted was lower for patients on daily haemodialysis than those on thrice-weekly haemodialysis. Moreover, daily haemodialysis was associated with higher risk of death, even after taking into account age and all major comorbidities.

“…In agreement, in the present French cohort, patients on early DHD were very different from patients on late DHD. Overall, French patients on early DHD were older and with several comorbidities, although we previously identified also a subgroup of young patients with high access to renal transplantation [16]. Therefore, in France, DHD is mainly used by two groups: 1) older and frail patients, presumably in response to cardiac instability and comorbidities, and 2) young patients to maintain their quality of life before kidney transplantation [15, 16].…”

“…Conversely, other studies reported that DHD is associated with better survival [1823]. These contradictory results could be explained by differences in the practices associated with DHD between France and other countries [16, 17].…”

“…Previous studies have shown improved survival with longer session durations only in patients receiving conventional 3 sessions/week . Whereas studies on the effect of increased session frequency, including the FHN trials, have compared frequent regimens that have varying session durations, including short duration, longer nocturnal sessions and more broadly intensive regimens that are some combination of more frequent or longer duration . This study separates the independent effects of increased session frequency and session durations to determine whether it is session frequency, session duration or both that is associated with improved survival in patients receiving more intensive HD regimens.…”

[5][6][7][8][9][10][25][26][27][28] This study separates the independent effects of increased session frequency and session durations to determine whether it is session frequency, session duration or both that is associated with improved survival in patients receiving more intensive HD regimens. Previous analysis of ANZDATA cohorts have suggested that it is the location of dialysis (home vs in-centre) that has been the main mediator of improved survival.…”
