Association between filaggrin null mutations and concomitant atopic dermatitis and contact allergy

Abstract: The spectrum of observable traits characteristic for the FLG mutation genotype in patients with AD is at present not defined. Our results indicate that the subset of patients with both AD and CA represent a phenotype of AD that is not associated with FLG mutations.

“…Furthermore, ADFM and ADWT showed similar impairments in cutaneous immunity to DNCB, suggesting that the immune dysregulation is not FLG dependent but is due to other AD-specific epidermal factors and may be protective against more inflammatory Th1 responses classically associated with ACD. These findings are in line with clinical observations reporting a lack of association between FLG mutations with a phenotype of patients with both AD and ACD (Carlsen et al, 2011). Nickel ACD has been reported to associate with the presence of FLG mutations (Thyssen et al, 2008), but others have only found a nonsignificant trend toward this finding (Carlsen et al, 2010).…”

“…11,17 However, other studies have found that irritant contact dermatitis and contact allergy are associated with FLG mutations but only in the presence of atopy. 32,33 Hence, we do not suspect that misclassification would be differentially related to FLG genotype. Finally, the participation rate was higher among the older age groups 34 but because age differences were not dependent on genotype, we do not consider this to affect the outcome of the study.…”

“…When genotyping and patch testing 430 Caucasians in Denmark, Carlsen et al did not fi nd a positive association between FLG mutations and contact sensitization [ 66 ]. Moreover, the subgroup of patients with AD and concomitant contact sensitization did not have a signifi cantly higher prevalence of FLG mutations compared with patients with positive patch test without AD, or compared with the general population [ 67 ].…”
