Psychosis is one of the most disabling psychiatric disorders. Paediatric traumatic brain injury (pTBI) has been cited as a developmental risk factor for psychosis, however this association has never been assessed meta-analytically. A systematic review and meta-analysis of the association between pTBI and subsequent psychotic disorders/symptoms was performed. The study was pre-registered (CRD42022360772), adopting random-effects model to estimate meta-analytic odds ratio (OR) and 95% confidence interval (CI) using the Sidik-Jonkman method. Subgroup (study location, study design, outcome type, assessment type, and adult verses adolescent onset) and meta-regression (quality of evidence) analyses were also performed. The robustness of findings was assessed through sensitivity analyses. The meta-analysis is available online as a computational notebook with an open dataset. We identified 10 relevant studies and eight were included in the meta-analysis. Based on a pooled sample size of 479,686, it was found that the pooled OR for the association between pTBI and psychosis outcomes was 1.88 (95% CI [1.07, 3.30]). There were no subgroup effects and no outliers identified. The association remained robust after removal of studies with low quality of evidence, however the OR reduced to 1.45 (95% CI [1.02, 2.07]). A leave-one-out sensitivity analysis showed the pooled association changed from marginally significant to marginally non-significant after removal of any one of three studies. In conclusion, we report cautious meta-analytic evidence for a positive association between pTBI and future psychosis. New evidence will be key in determining long-term reliability of this finding.

The rise of new media has greatly changed our lifestyles, leading to increased time spent on these digital platforms and less time spent reading. This shift has a particularly profound impact on children and adolescents, who are in a critical stage of brain development. Previous studies have found associations between screen use and mental health in children. However, it remains unclear whether these effects are mediated by other variables or if they directly cause the observed outcomes. Here, we utilized the ABCD dataset to examine the causal relationships between screen use and brain development (cognitive, behavioral, or neuroimaging outcomes), and tested the potential displacement effect of screen use on reading habits, based on Mendelian Randomization analysis. Our results revealed adverse causal effects of screen use on language ability and behavior (e.g. withdrawn/depressed symptoms and social problems) in early adolescents between 9 - 13 years old while reading had positive causal effects on their language ability and brain volume in the bilateral frontal and temporal regions. Interestingly, increased screen use was identified as a result, rather than a cause, of behavioral issues such as rule-breaking and aggressive behaviors. Furthermore, our analysis uncovered an indirect influence of screen use, mediated by changes in reading habits, on brain development, e.g., screen indirectly reduced brain volume by reducing reading time although no direct causal effect was found. These findings provide new evidence for a causal influence of screen use and reading habits on brain development and highlight the importance of monitoring media use and related habits change in children.

ImportanceChildhood exposure to mild traumatic brain injury (mTBI) is common. Individuals with a childhood history of mTBI experience more frequent criminal justice involvement in mid to late adolescence and adulthood. No study had been conducted to examine whether the link is causal or spurious.ObjectiveTo determine whether mTBI in childhood causes criminal justice involvement in mid to late adolescence.Design, Setting, and ParticipantsThis cohort study used population-based data for all children born between 1995 and 2000 in Denmark, with data linked to emergency department (ED) visits and hospitalizations before age 10 years and all criminal charges and convictions from ages 15 to 20 years. The exposure group contained all individuals diagnosed with mTBI before age 10 years without other intracranial or extracranial injuries; the comparison group was individuals not diagnosed with mTBI or intracranial or extracranial injuries. Sibling and twin fixed-effects models were used to evaluate the association after controlling for family-level confounding. Data were analyzed from May 2021 to July 2024.ExposuresMild TBI before age 10 years without other intracranial or extracranial injuries before or at the time of diagnosis.Main Outcomes and MeasuresAssociations between mTBI before age 10 years and criminal charges and convictions from ages 15 to 20 for the entire study population and separately by sex at birth, controlling for additional covariates.ResultsThe final analytic sample consisted of 343 027 individuals, 13 514 in the exposure group and 329 513 in the comparison group. Of the total sample, 166 455 (49%) were female and 176 572 were male (51%). A total of 326 191 participants (95%) had at least 1 parent with Danish citizenship, and 79 386 mothers (23%) held a college degree. There was a positive association between mTBI and criminal charges (odds ratio [OR], 1.26; 95% CI, 1.19-1.34) and convictions (OR, 1.24; 95% CI, 1.16-1.33). When controlling for family-level confounding, the associations became statistically insignificant and, in most models, greatly reduced. Results were robust across multiple model specifications.Conclusions and RelevanceThis study found that although mTBI in childhood was predictive of adolescent criminal justice involvement, there was no evidence that mTBI caused criminal charges or convictions.
