Association Between Plantar Fasciitis and Isolated Contracture of the Gastrocnemius

Abstract: Limited ankle dorsiflexion is commonly associated with plantar fasciitis and more than half of these patients had evidence of an isolated gastrocnemius contracture. These findings can be utilized to develop and further refine non-operative and operative treatment strategies for those with recalcitrant plantar fasciitis.

“…Various pathologies affecting the lower extremity, including plantar heel pain [1-3], metatarsalgia [4,5], stress fractures of the foot, and Achilles tendionpathy [6] are associated with limited ankle dorsiflexion (ADF). During gait reduced ADF results in an increase of forefoot pressure, which might be responsible for the above outlined pathologies [7-9].…”

“…The clinical test to assess isolated gastrocnemius tightness was described by Silfverskiöld. Isolated gastrocnemius tightness has been associated with failure of conservative measures [16]. Plantar fascia tension was directly proportional to Achilles tendon tension in cadavers in dynamic gait stimulator [17].…”

“…Daher wird die Einschränkung der DE als Ursache für eine Vielzahl von Krankheitsbildern, wie beispielsweise Achillodynien, Metatarsalgien und Plantarfasziitiden angesehen [7,9,13,21]. Der häufigste Grund für eine eingeschränkte DE ist die isolierte M. Gastrocnemiusverkürzung (MGV) [11,21].…”
