Association Between Rural Residence and In-Hospital and 30-Day Mortality Among Veterans Hospitalized with COPD Exacerbations

Abstract: Background We explored the relationship between rural residency and in-hospital mortality in patients hospitalized with COPD exacerbations. Methods We retrospectively analyzed COPD hospitalizations from 2011 to 2017 at 124 acute care Veterans Health Administration (VHA) hospitals in the US. Patient residence was classified using Rural Urban Commuting Area codes as urban, rural, or isolated rural. We stratified patient hospitalizations into quartiles by travel time from … Show more

“…This work was approved by the Institutional Review Boards and Research and Development Committee at the Iowa City VA Health Care System [IRB 201712713] as part of a larger study with a previous publication. 10 The study was conducted in accordance with the Declaration of Helsinki. A waiver of informed consent was granted for this retrospective study because the study examined only existing patient-level data.…”

“…In addition, our previous work, which used less stringent ICD code criteria, showed an accuracy in identifying AECOPD of between 80% and 90%. 10 We did not exclude patients with obstructive sleep apnea, as obstructive sleep apnea-COPD overlap is common, in particular among patients with advanced disease. 43 Excluding those patients would have resulted in a sample that was not a good representative of the true population.…”

“…The Rural Health Information Hub (RHIhub) has identified "geographic isolation, lower socioeconomic status, higher rates of health risk behaviors, limited access to healthcare specialists and subspecialists, and limited job opportunities" as significant risk factors for rural communities that directly play a role in the higher rate of chronic conditions for rural communities. 11,12 The purpose of this study is to investigate whether disparities in the rate of IP hospitalization exist in Missouri for three major chronic conditions (two specific and one nonspecific): 1) chronic kidney disease (CKD), 2) chronic obstructive pulmonary disease (COPD), and 3) obesity. That is, this paper seeks to assess whether variance in IP hospitalization rate for any of these three conditions exists and the degree to which residential status influences predicted variances while controlling for known disparities in condition prevalence.…”

“…That is, this paper seeks to assess whether variance in IP hospitalization rate for any of these three conditions exists and the degree to which residential status influences predicted variances while controlling for known disparities in condition prevalence. It is extremely important going forward to understand the difference between prevalence and IP 10 Liang et al, National Inpatient Hospital Costs: The Most Expensive Conditions by Payer, 2017 (2020) 11 RHIhub, Chronic Disease in Rural America (2019) 12 RHIhub, Rural Health Disparities (2019) hospitalization. Prevalence is the frequency of people in a defined population with a relevant condition, making prevalence rate the frequency with the condition divided by the entire population; while IP hospitalization is the frequency of people with this condition that were hospitalized in the IP hospital setting.…”
