Excessive gestational weight gain (GWG) is associated with increased risk of maternal and neonatal complications. We investigated obesity-related polymorphisms in the FTO gene (rs9939609, rs17817449) and ADRB2 (rs1042713, rs1042714) as candidate risk factors concerning excessive GWG in pregnant women with pregestational diabetes. This nutrigenetic trial, conducted in Brazil, randomly assigned 70 pregnant women to one of the groups: traditional diet (n = 41) or DASH diet (n = 29). Excessive GWG was the total weight gain above the upper limit of the recommendation, according to the Institute of Medicine guidelines. Genotyping was performed using real-time PCR. Time-to-event analysis was performed to investigate risk factors for progression to excessive GWG. Regardless the type of diet, AT carriers of rs9939609 (FTO) and AA carriers of rs1042713 (ADRB2) had higher risk of earlier exceeding GWG compared to TT (aHR 2.44; CI 95% 1.03–5.78; p = 0.04) and GG (aHR 3.91; CI 95% 1.12–13.70; p = 0.03) genotypes, respectively, as the AG carriers for FTO haplotype rs9939609:rs17817449 compared to TT carriers (aHR 1.79; CI 95% 1.04–3.06; p = 0.02).

Durante el embarazo, la alteración de la glicemia provoca una desregulación de la unidad fetoplacentaria, aumentando el riesgo de que tanto la madre como su hijo sufran complicaciones a corto y largo plazo debido a esta condición. Los criterios diagnósticos utilizados actualmente son tardíos, no existe consenso universal ni tampoco métodos predictivos que puedan pesquisar tempranamente la glicemia alterada (GA) en este periodo. Diversos estudios han demostrado que existen marcadores genéticos y clínicos que podrían servir como predictores de GA durante la gestación. Dentro de los genéticos, se han encontrado variantes en los genes TCF7L2, FTO y DIO2, los cuales están relacionados con un mayor riesgo a cursar con dicha condición. El objetivo del presente trabajo fue generar un modelo predictivo de GA en el segundo trimestre de embarazo, que incluyera las variantes genéticas mencionadas, sumado a datos clínicos y bioquímicos al inicio de la gestación. Para esto se recopiló información clínica y bioquímica de una muestra de pacientes embarazadas, quienes fueron clasificadas según sus niveles de glicemia (GA y glicemia normal (GN)), comparando ambos grupos. Estas pacientes fueron genotipificadas según los polimorfismos mencionados, para luego establecer las asociaciones individuales de cada genotipo con los parámetros clínicos y bioquímicos de las gestantes. Con esta información y utilizando el método multivariado Soft independent modelling of class analogies (SIMCA), se generó un modelo predictivo de GA en el segundo trimestre de embarazo, en base a los parámetros clínicos, bioquímicos y genéticos de relevancia del primer trimestre. Además, mediante un análisis de regresión por Partial least squares (PLS), se generó un modelo predictivo de glicemia a las 2 horas postcarga en el embarazo, utilizando variables de primer trimestre. Del total de pacientes embarazadas (68), un 35% (24) cursaron con GA durante la gestación, quienes se caracterizaron por ser de mayor edad y presentar mayores niveles de glicemia basal en el primer trimestre de embarazo. En el análisis genético no se encontraron asociaciones entre las variantes genéticas y la GA durante el embarazo. Sin embargo, para la variante rs7903146 (TCF7L2), el genotipo de riesgo “TT” se asoció con mayor peso e IMC en el primer trimestre y niveles elevados de glicemia basal en el segundo trimestre de embarazo, comparado con los otros genotipos. Además, los niveles de TSH fueron menores en las pacientes que presentaban el alelo de riesgo “T”. Para la variante rs225014 (DIO2), los niveles de glicemia basal del segundo trimestre fueron mayores en las portadoras del genotipo de riesgo “CC”, el cual también se asoció con niveles de triyodotironina total (T3T) disminuidos. También, se estableció una correlación positiva entre los niveles de glicemia a las 2 horas postcarga y el score genético, donde el mejor modelo (R: 0.35) consideró los genotipos rs12255372 (TCF7L2), rs9939609 (FTO) y rs225014 (DIO2). En el análisis multivariado, el modelo predictivo por SIMCA, para la clasificación de GA en embarazadas presentó un AUC (Area under curve) para la curva ROC (receiver operating characteristic) de 0.8734, junto con una sensibilidad y especificidad de 0.8182 y 0.9286 respectivamente, mientras que el modelo predictivo por PLS, para los valores de glicemia a las 2 horas postcarga en el embarazo, presentó un AUC de 0.7890 con la mayor sensibilidad y especificidad (0.7727 y 0.8333, respectivamente) en un corte de glicemia de 115 mg/dL. En base a los resultados expuestos, se puede evidenciar que los factores genéticos en conjunto con las variables clínicas y bioquímicas evaluadas son de gran relevancia para el desarrollo de GA durante el embarazo, y el modelo predictivo permitiría pesquisar tempranamente dicha condición.

Valid instrumental variables (IVs) must not directly impact the outcome variable and must also be uncorrelated with nonmeasured variables. However, in practice, IVs are likely to be invalid. The existing methods can lead to large bias relative to standard errors in situations with many weak and invalid instruments. In this paper, we derive a LASSO procedure for the k-class IV estimation methods in the linear IV model. In addition, we propose the jackknife IV method by using LASSO to address the problem of many weak invalid instruments in the case of heteroscedastic data. The proposed methods are robust for estimating causal effects in the presence of many invalid and valid instruments, with theoretical assurances of their execution. In addition, two-step numerical algorithms are developed for the estimation of causal effects. The performance of the proposed estimators is demonstrated via Monte Carlo simulations as well as an empirical application. We use Mendelian randomization as an application, wherein we estimate the causal effect of body mass index on the health-related quality of life index using single nucleotide polymorphisms as instruments for body mass index.
