Association of Implementation and Social Network Factors With Patient Safety Culture in Medical Homes: A Coincidence Analysis

Abstract: Objectives:The patient-centered medical home (PCMH) may provide a key model for ambulatory patient safety. Our objective was to explore which PCMH and patient safety implementation and social network factors may be necessary or sufficient for higher patient safety culture. Methods: This was a cross-case analysis study in 25 diverse U.S.PCMHs. Data sources included interviews of a clinician and an administrator in each PCMH, surveys of clinicians and staff, and existing data on the PCMHs' characteristics. We use… Show more

“…Second, research on social networks and their work-related outcomes is nascent in health care. Historically, the social network perspective—which encompasses theories, models, and applications that are expressed in terms of relational concepts or processes (Wasserman & Faust, 1994)—has predominantly been used in the health care setting to examine the impact of social networks on patient health outcomes and health care delivery (Dy et al, 2020; Yuan et al, 2020), with less emphasis on health workforce issues (Chambers et al, 2012). To our knowledge, this is one of only a handful of studies to examine whether health care workers’ social ties affect their job satisfaction (Tasselli, 2014) and the first to do so in the context of primary care.…”

“…CNA is a case‐based, mathematical approach to data analysis that draws on Boolean algebra, logic, and set theory to identify key conditions that uniquely distinguish a group of cases with an outcome of interest from another group without that outcome. Particular strengths of CNA include the following abilities: to model conjuncts (when several conditions must be present together to produce the outcome); to model disjuncts (when multiple pathways lead to the same outcome); to develop positive models (solutions for when the outcome is present) separately from negative models (solutions for when the outcome is absent); and to be applied to samples of different sizes, including small‐ n studies (Dy et al, 2022; Miech et al, 2021; Petrik et al, 2020; Whitaker et al, 2020; Yakovchenko et al, 2020). The analytic aim of configurational analysis is to identify necessary and sufficient conditions for an outcome of interest using the cases within a given dataset.…”

More recently, a 2019 Cochrane Review prominently used con gurational comparative methods to identify conditions aligned with successful implementation of school-based interventions for asthma selfmanagement [16]; a new comprehensive 2020 handbook on implementation science dedicated a complete chapter to CCMs [17]; and researchers have published numerous articles featuring CCMs in prominent health services journals in 2020, including BMC Health Services Research [18][19][20][21][22].…”
