Association of Oncologist-Patient Communication With Functional Status and Physical Performance in Older Adults

Abstract: IMPORTANCEThe functional status and physical performance of older adults with cancer are underassessed and undertreated despite the high prevalence of impaired functional status and physical performance in this population and their associations with chemotherapy-induced toxic effects and mortality. OBJECTIVE To examine the association between providing oncologists with a geriatric assessment (GA) summary with recommendations and having oncologist-patient conversations about functional and physical performance.

“…Excluding 13 patients (2.4%) who were missing audio recordings or had protocol violations (CONSORT previously published 45 ), an unadjusted 190 patients in the intervention group and 23 in the usual care group had any conversation with their oncologist about falls. As shown in Figure 1, 61.3% (95% CI, 50.9 to 70.8) of those in the intervention group had a conversation about falls compared with only 10.3% (95% CI, 6.0 to 17.0) in usual care ( P < .001).…”

“…This GA intervention has been shown to affect the quantity and quality of conversations regarding geriatric syndromes including polypharmacy, 55 comorbidities, 44 and physical function. 45 Implementation of GA in oncology practice settings has the potential to increase awareness of fall risk and fall risk management.…”

“…As a result of these potential advantages, the deployment of DHTs for capturing clinical research data has attracted attention from drug developers, academic researchers, regulators, and payers for more than a decade 21 . However, the extent of DHT adoption in oncology is somewhat behind other therapeutic areas, such as musculoskeletal and neurological disorders, 21 in part, due to the competing demands associated with managing patients on therapies with significant toxicities, and the fact that many patients suffering from cancer are older 4 . Currently, the most widely used applications of DHTs in oncology are electronic PROs (ePROs) and adverse event diaries administered electronically, along with wearable digital tools for estimating physical activity, and, to a lesser extent, vital signs and sleep parameters before, during, and after anticancer therapies.…”
