Association of Pulmonary Hypertension with Mortality in Incident Peritoneal Dialysis Patients

Abstract: ♦ Background: The prognostic value of pulmonary hypertension at the start of peritoneal dialysis (PD) in patient survival is unclear. ♦ Methods: We conducted a retrospective study of incident patients who initiated PD therapy from January 2007 to December 2011, and followed up through June 2013. Pulmonary hypertension was defined as an estimated systolic pulmonary artery pressure (PAP) of ≥ 35 mm Hg using echocardiography. Clinical parameters and laboratory findings were compared between patients with and with… Show more

“…Among the 18 articles included in the review, eight studies (10 525 participants) were retrospective and 10 (5273 participants) had a prospective cohort design. Five studies focused on non‐dialysis CKD populations (NKF‐KDOQI stages 1‐5ND), eight were conducted on hemodialysis and two on peritoneal dialysis populations. One study investigated a mixed (hemo‐ and peritoneal) dialysis cohort and another considered both pre‐dialysis and dialysis patients.…”

“…CVD was defined as a previous history of coronary artery disease, peripheral vascular disease or cerebrovascular disease. Pulmonary hypertension was defined as an estimated systolic pulmonary artery pressure (PAP) of ≥ 35 mm Hg using transthoracic echocardiography . Dialysis adequacy data, namely indices of urea kinetics (Kt/V) and creatinine clearance (Ccr) for both peritoneal and residual renal function (RRF) were determined using standard methods.…”

“…The ratio of peak early (E) and late (A) transmitral velocities was used to estimate left ventricular diastolic function. Left ventricular mass index (LVMi) was calculated using the Devereux formula . Systolic pulmonary artery pressure was calculated using the modified Bernoulli equation …”

“…Cardiovascular mortality was defined as death from ischemic heart disease, congestive heart failure, cerebrovascular disease, sudden death and peripheral vascular disease. Sudden death was diagnosed as unexpected natural death occurring within an hour from the onset of symptoms and without any prior condition that would appear fatal [15] . Survival time was defined as the time from enrollment to death or administrative censoring including renal transplantation, transfer to other dialysis centers, loss to follow-up or end of the study period (December 31, 2012).…”
