Association of Tumor Characteristics With Insurance Type Among Patients Undergoing Mohs Micrographic Surgery for Nonmelanoma Skin Cancer

Abstract: ohs micrographic surgery (MMS) is considered a standard of care treatment modality for nonmelanoma skin cancer (NMSC) of the head and neck. However, access to dermatologic care and MMS may be limited by wait times, cost, and the relative shortage of dermatologists in rural areas. 1 Insurance type is associated with increased stage at presentation, differences in treatment, and delays in care for patients with melanoma 2-4 and other noncutaneous cancers, including lung, prostate, breast, and colon cancer. “…29 These findings are consistent with a recently-published study by Clarke et al, which demonstrated that underinsured patients with NMSC undergoing MMS had significantly larger preoperative tumor bed sizes compared to privately insured patients. 30 Additionally, in lung and colorectal cancer, it has been demonstrated that patients who live farther from a cancer center are more likely to present with more advanced disease. 31 Ultimately, more advanced disease likely reflects both tumor aggressiveness and potential delay in seeking or receiving care, possibly due to provider availability or financial constraints.…”

“…Skin cancer remains the most common cancer in the United States 1,2 . For nonmelanoma skin cancer, which constitutes most skin cancers, Mohs micrographic surgery (MMS) can be performed by fellowship-trained dermatologists to remove tumor cells with intraoperative, microscopic confirmation of clearance 3 . The characteristics such as the size and depth of the defect created after MMS are difficult to predict and may require complex reconstruction, especially in cosmetically and functionally sensitive areas 4 .…”
