The importance of genetic factors in the genesis of dental caries of both primary and permanent dentitions is well established; however, the degree to which genes contribute to the development of dental caries, and whether these genes differ between primary and permanent dentitions, is largely unknown. Using family-based likelihood methods, we assessed the heritability of caries-related phenotypes for both children and adults in 2,600 participants from 740 families. We found that caries phenotypes in the primary dentition were highly heritable, with genes accounting for 54–70% of variation in caries scores. The heritability of caries scores in the permanent dentition was also substantial (35–55%, all p < 0.01), although this was lower than analogous phenotypes in the primary dentition. Assessment of the genetic correlation between primary and permanent caries scores indicated that 18% of the covariation in these traits was due to common genetic factors (p < 0.01). Therefore, dental caries in primary and permanent teeth may be partly attributable to different suites of genes or genes with differential effects. Sex and age explained much of the phenotypic variation in permanent, but not primary, dentition. Further, including pre-cavitated white-spot lesions in the phenotype definition substantially increased the heritability estimates for dental caries. In conclusion, our results show that dental caries are heritable, and suggest that genes affecting susceptibility to caries in the primary dentition may differ from those in permanent teeth. Moreover, metrics for quantifying caries that incorporate white-spot lesions may serve as better phenotypes in genetic studies of the causes of tooth decay.

Dental caries still remain one of the most prevalent infectious diseases, affecting populations and individuals unequivocally. It is observed that people with similar behavioral risk present different caries rates. Susceptibility to caries results from both the host and environmental factors that complement each other and may result in increased incidence of caries or greater resistance to it. Numerous studies of twins, families and animal models initiated in the early decades of the previous century provided strong evidence for the genetic component of dental caries development. The succeeding research, identifying specific genes and genetic markers and confirming the genetic correlation of dental caries, explained the reasons behind the greater susceptibility to caries in some diseases like Epidermolysis bullosa. Many inherited determinants are well established. The most recognizable are the structure of the hard dental tissue, tooth morphology including depth of occlusal fissures and surface characteristic of the enamel, time of eruption, alignment of the teeth in dental arches, flow rate of the saliva and its constituents, immunologic response to cariogenic microorganisms in oral cavity. An important role is also played by inherited thresholds for tastes and smells, which affect taste preferences and thus dietary habits and activity of specific enzymes participating in regulation processes on various stages of development and subsequent physiologic functioning of the organism. The knowledge of genetic basis of caries will enable us to identify high-risk groups and provide them with targeted screening, preventive measures and interventional strategies (Dent. Med. Probl. 2016, 53, 3, 413-418).

The purpose of this study was to evaluate the association between the frequencies of DR and DQ HLA class II alleles and the prevalence of dental caries in Brazilian adolescents. The study sample consisted of 164 adolescents aged 15– 19 years. For the assessment of caries experience the DMFT index was used, according to World Health Organization criteria. DNA samples of the adolescents were extracted from oral mucosa cells. The amplification of the alleles HLA-DR and HLA-DQ was carried out by polymerase chain reaction with sequence-specific primers (PCR-SSP). The products of PCR-SSP were electrophoresed and analyzed in agarose gel. Statistical analysis was performed using simple and multiple logistic regression to test the association between the adolescents’ caries experience and the presence of HLA-DR and -DQ alleles. Sociodemographic covariates were included in the model. The significance level for all analyses was p < 0.05. The prevalence of caries was 60.4% and the mean DMFT was 2.41 ± 2.53. Adolescents positive for HLA-DQ2 allele were less likely to have dental caries than their counterparts who were negative for this allele (OR = 0.33, CI 0.16–0.66), and those whose mothers presented low levels of education were 2.01 (CI 1.02–3.97) times more likely to have dental caries. There was no statistical difference between HLA-DR4, -DQ4, -DQ5, -DQ6 and dental caries. Our data provide evidence that genes within the MHC, especially the DQ2 group, may influence susceptibility to dental caries in Brazilian adolescents.
