Associations of Serum Cytokine Receptor Levels with Melancholia, Staging of Illness, Depressive and Manic Phases, and Severity of Depression in Bipolar Disorder

Abstract: To examine cytokine receptor biomarkers in bipolar disorder (BD), we recruited 133 well-phenotyped BD patients and 50 normal controls and measured serum levels of soluble interleukin 1 receptor antagonist (sIL-1RA), soluble interleukin-2 receptor (sIL-2R), sIL-6R, and tumor necrosis factor receptor 60 and 80 kDa (sTNFR60/80). sIL-1RA and sTNFR80 are significantly higher in BD than in controls and sTNFR80 and higher in melancholic than in non-melancholic patients and controls. Kapczinski's stages 3 + 4 are char… Show more

“…Recently, it has been reconceptualized as a changing disorder with different clinical manifestations over the course of its development from an at-risk or latent stage to a late or end-stage, thus supporting the assumption that it would benefit from a staging model 1 . Clinical manifestations consistent with staging models include cognitive deterioration and functional decline 2,3 , changes in inflammatory and neuroanatomical biomarkers [3][4][5][6][7][8][9] , less response to treatment 10 , and worse self-reported quality of life (QoL) 11 linked to disorder progression.…”

“…During depressive episodes, serum levels of CRP, TNF-α, IL-6, IL-1β, sTNFR1 and CXCL10 are elevated [ 10 , 47 , 51 ]. Increased depression severity is associated with greater elevations of pro-inflammatory cytokines [ 52 ]. During manic episodes, serum levels of IL-6, TNF-α, sTNFR1, IL-RA, CXCL10, CXCL11 and IL-4 are often elevated [ 47 , 51 ].…”

“…Recent emergent evidence points to the importance of the etiopathogenetic relationship between activation of immune-inflammatory pathways and psychiatric syndromes, including bipolar disorder (BD) (Berk et al, 2011;Grande et al, 2016;Hamdani et al, 2013;Leon-Caballero et al, 2015;Maes et al, 1995;Rege and Hodgkinson, 2013;Siwek et al, 2016). Available data support a relationship between autoimmunity and BD.…”
