Asymptomatic Carotid Surgery Trial-2 (ACST-2): Rationale for a Randomised Clinical Trial Comparing Carotid Endarterectomy with Carotid Artery Stenting in Patients with Asymptomatic Carotid Artery Stenosis

Abstract: Collaborators who routinely undertake CEA and CAS are encouraged to participate in ACST-2. This trial, now funded and open for randomisation, will provide important evidence comparing the immediate and long-term safety and efficacy of carotid endarterectomy and stenting in patients with asymptomatic carotid stenosis.

“…Since CAVATAS, multiple clinical trials have been performed or are in process comparing CAS to CEA for various indications (Andziak 2006;Brott et al 2010;CARESS Steering Committee 2003;Gaines and Randall 2005;International Carotid Stenting Study investigators et al 2010;Rudarakanchana et al 2009;SPACE Collaborative Group et al 2006;Yadav et al 2004). The Stenting and Angioplasty with Protection in Patients at High Risk for Endarterectomy (SAPPHIRE) trial randomized high-risk symptomatic (>50 % stenosis) and asymptomatic patients (>80 % stenosis) to CEA or CAS with distal protection (Yadav et al 2004).…”

“…The absolute risk reduction for endarterectomy is significant but numerically small at around 1% per year. 16,17 If results of the Asymptomatic Carotid Surgery Trial 2 (ACST2), 18 the Asymptomatic Carotid Trial (ACT1), 19 and the Stent Supported Percutaneous Angioplasty of the Carotid Artery versus Endarterectomy 2 (SPACE2) trial 19,20 show similar outcomes between carotid stenting and endarterectomy, two equally valid interventional options may exist. The last trial incorporates a third arm to address the benefit of contemporary BMT alone.…”

“…17 RESULTS Excluded studies. No data from the ongoing Stent-Protected Angioplasty in Asymptomatic Carotid Artery Stenosis vs Endarterectomy (SPACE2), 18 Asymptomatic Carotid Surgery Trial 2 (ACST-2), 19 and European Carotid Surgery Trial 2 (ECST-2) trials were available on request. Data from Stenting and Angioplasty with Protection in Patients at High Risk for Endarterectomy (SAPPHIRE) were excluded because this was a trial specifically for high risk for endarterectomy patients.…”
