Atraumatic surgical extrusion using periotome in esthetic zone: A case series

Abstract: Several techniques have been proposed for clinical crown lengthening procedures. Crown lengthening in esthetic zone can be classified in two basic types: Restorative and non-restorative cases after the crown lengthening is executed.Atraumatic surgical extrusion using a specially designed instrument called periotome was performed in four cases on isolated teeth in esthetic zone. The technique presented is an alternative approach to periodontal surgical procedures consisting orthodontic extrusion and osseous reco… Show more

“…However, these procedures are not without biological complications, including encroachment of the supracrestal tissue attachment (formerly biological width) compromising periodontal health, soft tissue relapse, a reduced crown-to-root ratio, loss of sound tooth structure owing to occlusal reduction of already compromised worn teeth – also resulting in insufficient retention and resistance for conventional extra-coronal restorations, and exposure of furcation and root dentine surfaces, which might invoke the unpleasant sensation of hypersensitivity if the tooth had been vital. 10,11…”

“…Multiple approaches exist for managing CRF: gingivectomy, apically positioned flap surgery, surgical or orthodontic extrusion, and extraction for extensive root involvement. However, these methods have limitations, particularly in esthetic zones [5] . Surgical extrusion, also referred to as intra-alveolar transplantation or intentional implantation, was initially introduced 24 years ago as a crown lengthening technique for CRF [6] .…”

“…Several modalities for the management of CRF are available: gingivectomy, apically positioned flap surgery, surgical extrusion, orthodontic extrusion and extraction when the fracture include more than a third of the root. 4 , 9 , 10 However, all these treatment modalities have some limitations especially when the esthetic zones are involved.…”
