Attention to emotional stimuli in borderline personality disorder – a review of the influence of dissociation, self-reference, and psychotherapeutic interventions

Abstract: Interactions between attention and processing of emotional stimuli shed light on both sensitivity to emotional stimuli as well as emotion dysregulation. Both of the latter processes have been proposed as central characteristics of altered emotion processing in those with borderline personality disorder (BPD). This review first summarizes the conflicting behavioural, psychophysiological and neuroimaging evidence for the hypothesis that emotional dysregulation should be reflected by higher distractibility throug… Show more

“…Other authors emphasized the importance of dissociation in the context of (social) cognition and BPD. For example, a recent review described associations between dissociation and task performance especially when interfering (emotional) stimuli are presented . On the brain level, dissociation‐induced impairments in working memory are accompanied by lower activity in the amygdala and other areas associated with emotion processing .…”

“…Emotional dysregulation is a typical symptom of BPD. Patients are more sensitive to emotional stimuli ( Winter, 2016 ) and frequently use maladaptive emotion regulation strategies, such as avoidance, impulsive behaviors, self blame and blaming others ( Fletcher et al, 2014 ). Impulsivity is another core feature of BPD patients, who often show impulsive behavior, such as suicide attempts, binge eating, substance abuse, and unprotected sex ( Lieb et al, 2004 ).…”

“…Importantly, frontal metabolism at baseline was predictive of clinical improvement (ZAN-BPD score decrease) (Ruocco et al, 2016). Clinical improvement associated with metabolism decrease in regions known to mediate sensory attention and emotion processing is consistent with higher attention to emotions characteristic of BPD (Winter, 2016). Finally, the association between decreased impulsivity and decreased thalamic metabolism highlight the proposed role of thalamus in decisional impulsivity (Wang et al, 2017).…”
