Atypical Hemolytic Uremic Syndrome following Acute Type A Aortic Dissection

Abstract: Atypical hemolytic uremic syndrome (aHUS) is a thrombotic microangiopathy (TMA)-related disease that manifests as a triad of microangiopathic hemolytic anemia, thrombocytopenia, and acute kidney injury (AKI) and is caused by uncontrolled activation of the complement system. We report the case of a 61-year-old woman with acute type A aortic dissection that subsequently developed into aHUS. The hematologic disorders underlying aHUS improved after treatment with the complement inhibitor eculizumab. It is importan… Show more

“…Device-related TMA as a secondary TMA could be difficult to diagnose if device implementation was performed years ago. Previous reports have demonstrated that cardiac valve-related TMA can appear soon after cardiac valve surgery [4,5], where it was diagnosed by excluding other TMA possibilities [6]. Here, we report the case of an older woman with a chief complaint of dyspnea and the pathophysiology of secondary TMA.…”

“…Although it is relatively easy to differentiate between STEC-HUS and TTP, the differential diagnosis of other TMAs is often difficult; all of these diseases are diagnosed as aHUS [5,9]. It is now widely recognized that aHUS diseases are complement-associated TMA [9].…”

“…Various studies concerning surgical interventions not related to transplantation have reported the development of TMA in the postoperative setting procedures [ 9 ]. Three case reports describe the new onset of aHUS as a differential diagnosis to AKI following cardiovascular procedures [ 10 , 11 , 12 , 13 ]. In a short period of time, several cases of TMA after a cardiovascular surgery were diagnosed at our institution.…”
