Atypical leg ulcers after sclerotherapy for treatment of varicose veins: Case reports and literature review

Abstract: HighlightsVaricosity treatment (related to arteriovenous malformation) results in skin ulceration.Skin necrosis is a serious but rare complication of sclerotherapy and healing period is usually prolonged.Wound care management vary from a non-adhesive dressing to LFUD/Surgical debridement.Awareness of this complication is essential since atrophic/hypertrophic scars are usually unavoidable and induce psychological trauma.

“…Long-term assorted follow-up prospective studies have been published [5] , [13] , and recording the following results for open surgery, EVLT, and USGFS of 94%, 90%, and 58%, respectively. Our data showed a high success rate of surgery and EVLT coinciding with previously published reports as regards the long-term follow-up [5] , [15] , [51] , [52] . In the current study, the success rate after adopting surgery and EVLT were approaching 91% and 90%, respectively.…”

“…Another patient suffered from leg ulceration 18 months after treatment with USGFS. The causative factor may be due to an increased concentration or extravasation of the sclerosing materials, as previously reported [51] . Despite the limitations and the low success rate of USGFS, it may still be considered as an effective and valuable option in treating some selected cases with varicose veins [4] .…”

“…To date, there is a paucity of published literature describing the phenomenon of cutaneous necrosis as a complication of sclerotherapy. A group in Perth, Australia, has reported two cases of atypical leg ulceration following sclerotherapy 10 . However, our case is unique due to the delayed presentation, which is atypical of intra‐arterial sclerosant injections, and the extensive nature of skin and cutaneous involvement.…”

The exact mechanism of necrosis in our patient is unclear as no extravasation or intra-arterial injection of polidocanol was seen at the time.…”
