Atypical metanephric adenoma – a case report and review of literature

Abstract: Metanephric adenoma (MA) is a rare renal neoplasm that generally occurs in adults and is considered to have a good prognosis. However, one case of MA with atypical histological features and distant metastasis and another case of metastasizing MA with typical cytologic features have been reported. The authors here report a case of MA with atypical histological features arising in the left kidney of an 18-year-old girl presenting with painless hematuria. Relevant literature is reviewed and discussed.

“…Most of the MA-like tumors are detected incidentally by diagnostic imaging, without showing clinical findings. CT scans, ultrasound and MRI imaging do not usually further elucidate a specific image diagnosis von Willebrand disease have been found in patients with MA [6]. However, no other MA has been reported in patients with nonHodgkin lymphoma diagnosed, as in our case.…”

“…In the last 14 years only 2 cases of MA have been found among more than 100,000 biopsies reported at the Hospital Universitario Miguel Servet, Zaragoza, Spain. Some cases of MA have been described in the literature associated with hematological diseases, particularly polycythemia Vera [6,8]. There is a higher incidence of simultaneous polycythemia compared to any other renal diseases.…”

“…However, the current case demonstrated a clearly defined margin with diffuse calcifications, dissimilar to that observed in previous cases. This formation of diffuse calcifications may correlate with intratumoral hemorrhage (12,13). Patients with metanephric adenoma typically have a good prognosis after undergoing total nephrectomy or local resection with kidney preservation (14).…”

“…17 In our initial experience, MA is a heterogeneous neoplasm with different pathological and biological characteristics; similar findings include metastatic cases reported in English literature. 18,19 Theoretically, CT findings, especially enhancement, may reflect the histological composition, and the variable enhancement degree concerning MA should suggest the possibility of coexisting subtypes. Correspondingly, tumour CT attenuation values and enhancement ratios were correlated with histological subtypes.…”
