Auditory Nerve Recovery Function in Cochlear Implant Surgery with Local Anesthesia and Sedation versus General Anesthesia

Abstract: Objective: To analyze auditory nerve recovery function (REC), comparing the results after local anesthesia and sedation with those after general anesthesia. Methods: This was a prospective, cross-sectional and descriptive study in a tertiary referral center hospital. We included all patients (37 ears) who underwent surgery for cochlear implant under either general anesthesia or local anesthesia and sedation during 3 years. REC was measured according to the function of 3 components, ‘T0', ‘A' and ‘tau', and was… Show more

“…These results are consistent with the absolute refractory period estimated based on single-fiber recordings in animal studies (e.g. Bruce et al, 1999; Dynes, 1996) and the results measured in adult CI users (Morsnowski et al, 2006; Wiemes et al, 2016). In comparison, t 0 estimated in children with CND at all test sites were significantly longer than those estimated in children with normal-size CNs.…”

“…Time constants, including t 0 and τ, derived from the refractory recovery function are important measures of the CN’s refractory properties (e.g. Botros & Psarros, 2010; Morsnowski et al, 2006; Wiemes et al, 2016). Procedures used to derive these time constants are reported in the Data Analysis session.…”

“…Botros & Psarros, 2010; Fulmer et al, 2011; Morsnowski et al, 2006) was 1.00 and 1.01 ms, respectively. These values are within the range reported for typical CI users (He et al, 2017; Morsnowski et al, 2006; Wiemes et al, 2016). In contrast, the eCAP could only be recorded when the interval between the masker and the probe (i.e.…”

“…To address the needs of patients who have multiple medical problems that significantly increase the risk of GA or who harbor a fear of GA, CI surgery under local anesthesia and conscious sedation (LA‐CS) has been offered. The literature to date on CI under LA‐CS has been limited to case reports and small case series but demonstrates the feasibility of this technique 14–23 . This includes a publication from our center comparing small age‐matched cohorts of patients who underwent CI under LA‐CS and GA 14 .…”
