Background:-Thyroid gland is one of the important organ in human body and the burden of thyroid diseases in the general population is enormous specially in females. In Saudi Arabia its pattern is differ from area to area Aim of the study:-The present work was conducted to study the pattern of thyroid disorders among patients attended to the endocrinology clinics in King Khalid Hospital , Hail region , Saudi Arabia. Results:-The patients complaining of different thyroid disorders were 175. 98(56 %) cases was found to have hypothyroidism and 57 (32.6 %) had hyperthyroidism. Iodine deficiency was associated with hypothyroidism in females but it was common in males with hyperthyroidism The etiology and risk factors of hypothyroidism were stress, malignant tumors, Hashimooto thyroiditis and diabetes in males while they were bad nutrition, Iodine deficiency, goiter, benign thyroid tumor and family history in females. Diabetes , graves disease , benign and malignant tumors were common in males but bad nutrition , iodine deficiency , goiter and family history were the more frequent in fameless suffered from hyperthyroidism .Total cases with goiter were 89 most of them were males (60.7%) palpable goiter was more common 75.5 than visible type. Malignant thyroid were found mainly in males (75%) with papillary type of cancer (64.3%) and the common presenting manifestation was hyperthyroidism (53.6%). Conclusion:-Thyroid disorders are common in Hail region specially in females .The pattern is more or less similar to that detected in other Saudi cities .Goiter and malignant thyroid are more common in males. Recommendation:-More studies should be carried out in Hail region to stress on the individual thyroid disorder so as more comprehensive studies could be obtained.

Objective: Autoimmune pathologies are a growing aspect of medicine. Knowledge about atypical cases is essential. This report will describe a case of unusual, alternating fluctuations in thyroid function. Methods: We report a case of thyrotoxicosis alternating with hypothyroidism in a 44-year-old, African-American woman and detail the clinical course and management. Results: The patient presented in a mildly thyrotoxic state with features of thyroiditis that resolved soon thereafter. Subsequently, the course shifted toward a hypothyroid state with a thyroid-stimulating hormone (TSH) level of 24.53 µIU/ml (normal range is 0.45 to 4.5 µIU/ml; measured September 5, 2013) and free thyroxine (FT4) of 0.35 ng/dL (normal range is 0.5 to 1.40 ng/dL; measured September 5, 2013). It ensued with alternating hypothyroid and hyperthyroid trajectories for several cycles. Clinical management was adjusted to negotiate each progression. During certain intervals, levothyroxine was increased. At other visits, it was decreased. Periods without medication were observed as well. Furthermore, methimazole and metoprolol were utilized when required. Reversal of the condition occurred repeatedly. The entire course is tracked with over 30 instances of thyroid function measures that included hypothyroid, euthyroid (TSH at 1.54 µIU/mL, FT4 at 1.16 ng/dL) and thyrotoxic states (TSH at <0.005 µIU/mL, FT4 at 2.67 ng/dL). Various antibody titers were elevated including thyroid-stimulating immunoglobulin, thyroid peroxidase antibody, and TSH receptor antibody. Close monitoring of TSH and FT4 allowed for appropriate medication dose adjustment. Conclusion: This case highlights the unusual phenomenon of fluctuating thyroid function with autoimmune involvement of thyroid-stimulating immunoglobulin and TSH receptor antibodies. Close follow up aided responsive clinical management throughout the fluctuating clinical course. (AACE Clinical Case Rep. 2019;5:e112-e118

Abstract:Subclinical thyroid dysfunction is defined as an abnormal serum thyroid stimulating hormone level and free thyroxine and triiodothyronine levels within their reference ranges. We report here a case of otherwise asymptomatic subclinical hypothyroidism, who progressed to overt hyperthyroidism within a span of one year, with tremors, heat intolerance and fidgetiness. If a patient on levothyroxine for autoimmune hypothyroidism on replacement therapy presents with hyperthyroidism, the possibility of endogenous hyperthyroidism should also be considered.
