Autoimmunity and infection in Sjögren's syndrome

Abstract: It is widely proposed that microbial factors may incite autoimmune disease. While this hypothesis is proven in a few illnesses such as rheumatic fever, there is no definitive evidence of an infectious environmental trigger in Sjögren’s syndrome. However, there are circumstantial data with regard to viruses and several potential mechanisms of disease. These include antigen mimicry, polyclonal lymphocyte activation and infection mediated innate end-organ inflammation. In addition, hepatitis C virus infection cle… Show more

“…Antigen mimicry, induced by aberrant EBV exposure or defective immune control, plays a central role in each disease and has been addressed in a number of recent reviews (Ballandraud et al 2004;Pender 2011;Lossius et al 2012;James and Robertson 2012;Füst 2013;Igoe and Scofield 2013). All affected individuals, including the juvenile cases, are EBV seropositive (i.e.…”

“…Patients with MS also have elevated levels of serum IgG Ab against many EBV Ags (35). EBV may also be involved in the salivary gland tissue damage leading to Sjögren's syndrome (36). Data suggesting this possibility include a higher incidence of EBV reactivation in Sjögren's syndrome patients, as well as increased levels of EBV Ag and DNA in salivary-infiltrating lymphocytes (37).…”

“…It mainly affects middle-aged women with a female to male ratio ≥ 9:1 [1] [2]. A large number of articles refer to the role of several infections in the pathogenesis of SS and the influence of these infections as a trigger for SS symptoms [3] [4]. However, few of these articles mention the susceptibility of these patients to new infections other than intraoral or ocular infections.…”

“…However, few of these articles mention the susceptibility of these patients to new infections other than intraoral or ocular infections. Patients with SS have defects in their immune system, such as acquired or inherited complement deficiencies [4] [5], and frequently use immunomodulatory drugs, all of which predispose to bacterial infections. Infections caused by encapsulated organisms or tuberculosis may be those posing a higher risk for SS patients [3] [5] [6].…”
