Automated image analysis of hand radiographs reveals widened joint spaces in patients with long-term control of acromegaly: relation to disease activity and symptoms

Abstract: Objective: Arthropathy is an invalidating complication of acromegaly. Although acromegalic arthropathy shares features with primary osteoarthritis, joint spaces are widened rather than narrowed in patients with long-term cure of acromegaly. The late effects of acromegaly on hand joints have not been characterized. Therefore, the objective of the current study was to assess joint space widths (JSWs) of hand joints in patients with long-term control of acromegaly and to identify factors associated with JSW. “…New techniques are needed to measure bone and joint integrity to assess better the effects of acromegaly treatment (MQ). [45][46][47][48] Different medications for acromegaly have different effects on glucose metabolism. SRLs might have a negative influence on glucose metabolism, but generally with marginal clinical relevance and most frequently in patients who do not have biochemical control of the disease.…”

“…Recently, van ‘t Klooster and colleagues [70,71] have introduced a new quantitative semiautomated image analysis method of hand radiographs, which is sensitive enough to relate joint space width to markers of disease activity of acromegaly. Therefore, this method is able to fully characterize the effects of previous GH excess on joint cartilage, including the measurement of joint space widening.…”

“…Further investigation of this issue may be necessary. Most publications qualitatively document the swelling at mouse paws through radiographs of the inter-phalangeal joints or by histological exam [34-36] but little or no quantitative analysis. Here, we present a semi-quantitative method allowing us to compare levels of swelling at mouse paws after different treatments.…”
