Autotransplantation of a maxillary first premolar to replace an ankylosed maxillary incisor: 7-year follow-up

Abstract: • Autotransplantation is a viable option for the treatment of a missing tooth or for the replacement of a traumatized tooth when there is a donor tooth available. • Autotransplantation of a premolar for replacement of a missing anterior tooth is sometimes a suitable alternative to conventional prosthetic rehabilitation or implant treatment in young individuals. • Proper combined endodontic and orthodontic treatment of autotransplanted teeth might be possible without periodontal complications.

“…The use of calcium hydroxide as an intracanal dressing is required as part of the endodontic treatment because the high pH and antimicrobial properties of this compound may contribute to the prevention of root resorption and stimulation of the healing process (9,10,13,17). In this case, a calcium hydroxide dressing was used in addition to the already described properties to stimulate the process of apical closure.…”

“…Autotransplantation is defined as the transplantation of teeth from one site to another in the same individual into extraction sites or surgically prepared sockets (10). The advantages of autotransplantation include the possibility of maintaining the viability of the periodontal ligament, orthodontic movement and proprioception, and preservation of the alveolar bone and gingival graft with a natural contour.…”

“…Dharmani et al (53) presented a case of a 17-year-old male 1-week post-trauma to tooth 12 where a horizontal root fracture was diagnosed and given a poor prognosis. This group noted the need to surgically resect the buccal root of the premolar, fill the resected root with composite, dress the palatal root with calcium hydroxide for 2 months, complete root canal treatment, and then initiate orthodontics (54). Upon a 2-year recall, the tooth was judged esthetic and functional (53).…”

“…Tsurumachi and Kuno (54) discussed autotransplantation of a maxillary premolar after extraction of an ankylosed incisor. This group noted the need to surgically resect the buccal root of the premolar, fill the resected root with composite, dress the palatal root with calcium hydroxide for 2 months, complete root canal treatment, and then initiate orthodontics (54). The predictability of autotransplantation has been further discussed regarding its acceptable esthetic and clinical surrogate measure outcomes by Czochrowska (55)(56)(57).…”
