Availability of Pediatric Inpatient Services in the United States

Abstract: OBJECTIVES:We sought to evaluate trends in pediatric inpatient unit capacity and access and to measure pediatric inpatient unit closures across the United States. METHODS:We performed a retrospective study of 4720 US hospitals using the 2008-2018 American Hospital Association survey. We used linear regression to describe trends in pediatric inpatient unit and PICU capacity. We compared trends in pediatric inpatient days and bed counts by state. “…The significantly higher number of admissions to WCH and less severe case-mix are likely a reflection of infrequent regional hospitalization prior to transfer. The lack of regional care is an increasing trend in North America and leads to transfer as a rule, with loss of regional pediatric capacity and little room to surge [ 27 ]. SCH and CHEO’s transfers generally hail from larger centers, Stanton Territorial Hospital (STH) in Yellowknife and Qikiqtani General Hospital (QGH) in Iqaluit, that have benefitted from improved access to on-site care from full-time pediatricians.…”

“…For those with lower socioeconomic status such a distance can be quite substantial in the potential absence of having access to a car, gasoline prices, and lack of public transportation. Effects of distance on access to pediatric cardiac care have been demonstrated by several studies [17]. Those with public insurance and higher deprivation index have been documented to be more likely to be able to complete pediatric cardiology visits, with up to 40% of new referrals not even being completed (Warren).…”

“…While general (ie, non-paediatric) EDs’ paediatric readiness has been increasing in the USA, hospitals’ capability to provide definitive paediatric care has been decreasing 16. This trend has been due in part to inpatient unit closures and has led to increased ED transfers of children, even for common, basic conditions such as asthma and gastroenteritis 17 18…”
