Axial Length of the Eyeball Is Important in Secondary Dislocation of the Intraocular Lens, Capsular Bag, and Capsular Tension Ring Complex

Abstract: Purpose. To analyze the patients with secondary dislocation of CTR and IOL within 5 years from cataract surgery, to determine predisposing factors. Methods. 16 eyes of 15 patients aged 66.2 ± 6.7 (from 49 to 82) with CTR/IOL complex dislocation within 5 years from cataract surgery were compared with 26 patients aged 67.1 ± 7.2 (from 53 to 85), implanted with CTR during cataract surgery to manage zonule dehiscence and did not dislocate for at least 5 years, in respect of cause, axial length and IOL power, refra… Show more

“…To our knowledge, this is the first case series on the retropupillary iris fixation of an iris claw Artisan Myopia IOL (model 204) in patients with high myopia who present with IOL dislocation. A longer eyeball in eyes with high myopia is strongly associated with dislocation of the IOL-bag complex [12]. A previous study reported the need for IOL exchange in children with pseudophakic eyes because of myopic shift resulting from axial elongation [13].…”

“…Activities such as lifting of heavy objects could increase intraocular pressure and push the IOL and IOL haptic anteriorly, hence the defect of the IOL haptic. Klysik et al [10] stated that axial length and low IOL power could be the risk factors of lens dislocation within 5 years of surgery and that the axial length is statistically significant in the incidence of IOL dislocation ( p = 0.009). Longer axial length also contributes to a higher risk of IOL dislocation.…”

“…Применение внутрикапсульных колец позволяет расправить капсульный мешок и создать равномерное растяжение капсульной сумки, однако данные о профилактике ККС в таких случаях противоречивы. Не исключается и риск спонтанной дислокации блока «капсульный мешок -ИОЛ -внутрикапсульное кольцо» в стекловидное тело [3].…”
