“…Another study showed that elevated expression of EV-miR-15a in plasma is correlated with initial retinal damage, suggesting its potential application in the early prevention of DR in patients with T2DM 87 . Similarly, miR-3976, miR-431-5p, and miR-26b-5p derived from serum EVs have been reported to be up-regulated in patients with DR 67 , 88 , 89 , indicating their potential utility in monitoring the progression of DR. Tears, a valid source of biomarkers for ocular diseases, show aberrant proteomics or metabolomics alterations in DR 90 , 91 . Moreover, EVs from tear samples have been isolated and performed transcriptomic analysis, which disclosed the upregulated miR-145-5p, miR-214-3p, and miR-9-5p and the downregulated miR-146a-5p, miR-31-5p, and miR-96-5p in diabetic patients with retinopathy compared with the control 92 .…”

“…The investigation of biomarkers from the AH and VH provides a deeper understanding of ocular diseases, including Glaucoma [ 50 ], Congenital Ectopia Lentis [ 51 ], and Diabetic Retinopathy [ 52 , 53 ]. Nonetheless, tears have been the non-invasive biological specimen of choice to find potential biomarkers in systemic diseases such as Type 2 diabetes mellitus (T2DM) [ 36 , 54 ], and ocular disorders (Keratoconjunctivitis [ 55 ] and Keratoconus [ 56 ]).…”

“…Furthermore, retinopathy can serve as a biomarker for vascular disease risk in asymptomatic diabetic individuals, indicating an elevated risk of life-threatening systemic vascular complications [ 126 ]. In a recent study [ 113 ], GC-MS was employed to analyze tear fluid samples from 41 T2DM patients with DR as well as 21 non-diabetic patients. The study revealed a significant prominence of D-glutamate and its metabolism in the proliferative diabetic retinopathy (PDR) group compared to the non-diabetic group.…”
