B:2a:P1.5 Meningococcal Strains Likely Arisen from Capsular Switching Event Still Spreading in Spain

Abstract: Eighteen clustered cases of meningococcal disease associated with B:2a:P1.5 strains doubled the annual incidence up to 4.3 ؋ 10 5 in Navarra, Spain, in 2007. Eleven percent of cases were fatalities, and 74% of cases were individuals 10 to 24 years old. This is the third cluster associated with this strain in northern Spain since 2001.

“…This sequence has subsequently been found in a large group of isolates of the B:2a:P1.5 phenotype, mostly isolated in Spain from 2002 to 2008, and belonging to the ST-11 cc. These may have arisen by horizontal transfer of the capsular operon (36). In the majority of isolates analyzed, the nadA gene was inactivated by frameshift mutations.…”

“…11 Among the targeted age groups, the incidence of meningococcal C disease decreased conjugated meningococcal C vaccine. 9,10 In contrast, capsular switching has not been observed following such immunization in England. 11 Thus, potential replacement of N. meningitidis serogroups following wide acceptance of the quadrivalent (serogroups A, C, W-135, Y) conjugate vaccine may occur and should be monitored.…”

“…Will vaccination drive capsular switching (e.g., serogroup C to B or other nonvaccine serogroups) as was seen with pneumococcal vaccines 80 and which has been suggested in some clinical isolates following the introduction of routine meningococcal C vaccination. 81,82 What immunization schedule is the most effective and cost-acceptable? This last issue has been the recent subject of debate.…”

“…The L2 Spanish strains described in the present study are B:2a:P1.5 isolates. It is likely that they derive from the Spanish meningococcal serogroup C:2a:P1.5 strain, after capsular switching event from serogroup C to B [30,31]. Some of these Spanish C:2a:P1.5 isolates were characterized by the presence of the insertion sequence IS1301 in the capsular locus, between the sia and ctr operons.…”
