Endovascular Repair of an Aorto–Left Renal Vein Fistula Due to a Ruptured Abdominal Aortic Aneurysm After EVAR

Abstract: This case emphasizes the need of close surveillance even in the late postoperative course of these patients. Moreover, this rare event confirmed that endovascular techniques can play an important role in treating emergent complications.

“… 7 However, only one report described a late complication of EVAR with an arteriovenous fistula between the aneurysm sac and a RLRV after a type III endoleak with AAA expansion. 3 To our knowledge, this work is the first report of ALRVAF owing to type a IB endoleak.…”

“… 2 Even rarer, an arteriovenous fistula between the aneurysm sac and a retro-aortic left renal vein (RLRV) after a AAA type III endoleak expansion has been described only once. 3 This report describes to a patient who developed an aorto-left renal vein fistula owing to a type IB endoleak 2 years after endovascular aneurysm exclusion. The leak was repaired with a left endograft limb extension.…”

“…Endovascular repair has been used for the management of primarily ruptured AAAs, with promising results [4][5][6][7][8] ; however, such an approach has been infrequently reported in cases of secondarily ruptured AAAs after EVAR. 2,9 Indeed, in emergent situations, issues such as the availability of experienced staff or appropriate stent graft dimensions may limit its feasibility. Difficulties during endovascular repair of secondarily ruptured AAAs after EVAR can also arise owing to the incompatibility of different device characteristics.…”
