Baby-led weaning: what a systematic review of the literature adds on

Abstract: The term weaning describes the time period in which a progressive reduction of breastfeeding or the feeding of infant-formula takes place while the infant is gradually introduced to solid foods. It is a crucial time in an infant’s life as not only does it involve with a great deal of rapid change for the child, but it is also associated with the development of food preferences, eating behaviours and body weight in childhood and also in adolescence and adulthood. Therefore, how a child is weaned may have an infl… Show more

“…First developed in 2007 (as AMSTAR) to only evaluate the methodological quality of SRs that synthesize evidence from randomized trials, this appraisal tool has been further developed, as AMSTAR 2, in 2017 to expand its use to SRs of randomized trials and non-randomized studies 23. Since its release, AMSTAR 2 has been used widely in many umbrella reviews 27–33. AMSTAR 2 comprises 16 domains, 7 of them are critical domains as they strongly undermine the confidence in the conclusions of the SRs: 1 domain is related to protocol registration, 2 are related to search strategy (adequacy and justifying studies’ exclusion), 2 are related to the assessment of risk of bias of the included studies and its effect on SR conclusions, 1 is related to the method of evidence synthesis, and 1 is related to the publication bias (table 1).…”

“…Within BLW, parents wait to introduce complementary foods and beverages until their infant exhibits developmental signs of readiness to self‐feed (ie, ability to sit‐up unsupported, with good head and neck control), which generally occurs around 6 months of age. While BLW is increasing in popularity, research on this approach is limited and is confounded by the fact that many children who primarily self‐feed may also be spoon‐fed to some extent by caregivers 39 . Some evidence for potential benefits of BLW on infant eating outcomes (such as improved satiety responsiveness in later toddlerhood 40 ) exists, as well as evidence that BLW may have other benefits for the family, such as lower maternal anxiety related to feeding 41 .…”

“…Nevertheless, fish contains important nutrients and omega-3 fatty acids play an important role in development of the central nervous system (47,48). We feel reasonable to recommend fish introduction during the second semester of life, timing based on local tradition, weaning approach, and family preferences (49,50).…”
