Barrett's oesophagus patients with low-grade dysplasia can be accurately risk-stratified after histological review by an expert pathology panel

Abstract: Confirmed LGD in BO has a markedly increased risk of malignant progression. However, the vast majority of patients with community LGD will be downstaged after expert review and have a low progression risk. Therefore, all BO patients with LGD should undergo expert histological review of the diagnosis for adequate risk stratification.

“…Review by two GI pathologists, with extensive experience in the diagnosis of BE-related neoplasia, found that of 147 patients diag nosed with LGD in the community, 85% of the patients were downgraded to a diagnosis of no dysplasia ( 122 ). Further work by that group examined 293 additional patients with LGD diagnosed in the community who had biopsies reviewed by at least 2 GI pathologists and 73% of the cases were downgraded to indefinite for dysplasia or nondysplastic BE ( 123 ). Other studies suggest that community-based pathologists have diffi culties in the interpretation of both nondysplastic BE and dysplasia ( 124 ).…”

“…29 Similarly to the aforementioned study, they demonstrated that only a small percentage (27%) of the original LGD diagnoses were confirmed, while the remaining 73% of cases were down-staged. After a median follow-up of 39 months the risk of progression to HGD or OAC was 9.1% per patient-year in the confirmed LGD group.…”
