Baseline CD4 T Cell Level Predicts Recovery Rate after Initiation of ART in HIV Infected Nigerians

Abstract: The most characteristic immunologic disorder in HIV infection is the progressive loss of CD4 T lymphocytes, thus, it remains the most important and commonly used marker for monitoring of immune status of HIV-infected individuals. This study monitored CD4 T lymphocyte cell dynamics among HIV patients on ART, and consequently defined an optimal baseline level required for enhanced ARV treatment. Ninety-eight (M = 33; F = 65) out of 106 consenting HIV-infected ARV-naïve patients enrolled and monitored for 24 mont… Show more

“…This study helps to identify the stage of the disease at which the majority of HIV individuals present voluntarily for care and treatment. Previous studies have highlighted the significance of baseline CD4 levels in predicting immune recovery and clinical outcome [ 8 ]. Studies investigating normal lymphocyte ranges including CD4+ cells have also been used to guide HIV treatment protocols in countries such as Turkey and Nigeria [ 12 , 15 ].…”

“…The results presented show a picture of the immune status of a representative urban Nigerian population at the time of first voluntarily HIV testing. We therefore recommend an upward revision of the ART enrolment criteria from the 250 cells/µl to at least 350 cells/µl, which is currently being advocated [ 7 , 8 ]. Also, community engagement should be intensified in order to encourage regular and universal counseling and testing across gender and age groups in order to enable early HIV detection and to facilitate better immune recovery among HIV-positive Nigerians.…”

“…The past studies have also demonstrated that treatment outcome and CD4 cell recovery rate can be driven by the time at which care, support, and treatment is started 17,18 . Therefore, understanding the baseline CD4 count of the diagnosed treatment naïve PLHIV is very important.…”
