Basics of prevention and management of iodine-based contrast media-induced thyroid dysfunction — position paper by the Polish Society of Endocrinology

Abstract: Medical practice involves a high number of radiological examinations using iodinated contrast media (ICM). Therefore, it is crucial for doctors of different specialties to be aware of possible adverse effects associated with ICM use. The most common and well characterized adverse effect is contrast-induced nephropathy, whereas thyroidal adverse reactions remain a diagnostic and therapeutic dilemma. ICM-induced thyroid dysfunction represents a highly heterogenous group of thyroid disorders. Due to supraphysiolo… Show more

“…The preferred method is laparoscopy due to the lower invasiveness of the procedure, but the presence and nature of metastatic lesions most often require modification of the procedure to laparotomy. It was indicated that the limiting tumor size for laparoscopic surgery is 6 cm [18,36,37]. A different survival rate was reported by De Filpo et al which described currently surgical resection of a metastasis--free tumor using available imaging technologies at the time of initial advancement results in a 5-year survival rate of more than 90% [38].…”

“…Malignant lesions are further confirmed by high contrast > 30 IU H (i.e. low fat content) and delayed contrast washout (less than 50% after 10 min) [2,[9][10][11][12].…”

“…It accounts for 5% of all adrenocortical tumours (ACTs). There are 2 peaks of incidence: in the first decade of life and between 40 and 50 years of age [1][2][3]. Based on an analysis of the literature on adrenocortical carcinoma, this article presents the diagnosis, treatment modalities, genetic background, and prognostic factors of ACC in children.…”

“…This supports the view that this type of lesion does not require follow-up. In contrast, other guidelines ( 8 ) or consensus positions ( 9 11 ) recommend maintaining radiological and biochemical follow-up of NFAT, regardless of their characteristics, due to the possibility of changes in their nature when are followed for a long period. In fact, relatively few studies really addressed long-term follow-up of unresected AI, making the usefulness and possible timing of reassessment over time unclear.…”
