Basosquamous carcinoma: appearance and reality

Abstract: Basosquamous carcinoma (BsC) is a controversial entity and both a diagnostic and therapeutic challenge. BsC has mixed histopathological characteristics of both basal cell carcinoma (BCC) and squamous cell carcinoma (SCC). BCC and SCC display characteristic histopathology and behaviour; on the other hand, BsC is a rare tumour, which has variable morphology and displays less predictable behaviour. An early diagnosis of BsC is important due to the particularly aggressive nature of the tumour, the increased likeli… Show more

“…Due to experience and research with both BCCs and SCCs, surgical excision is currently the first-line treatment option, but a significant role has been demonstrated for radiotherapy and Mohs' micrographic surgery [ 5 , 11 , 21 24 ]. To date, there is no standardised treatment protocol for the management of basosquamous carcinoma [ 25 ].…”

“…In this cohort, subsequent treatment varied widely. Previous reports have demonstrated positive margins in more than 30% of cases following primary excision, usually presumed due to the microscopic invasiveness of the disease, and this is well documented to be a significant risk factor for recurrence of disease [ 4 , 7 , 25 , 30 ]. The most likely reason for this low incidence is the generally low clinical staging of disease, but this has yet to be tested and evaluated through further research.…”

“…Clinically, BSC is indistinguishable from BCC and is commonly referred to as "metatypical basal cell carcinoma [122,123] ". Diagnosis of BSC is typically made with a biopsy of a lesion suspected of being a BCC or an SCC [124] . It should be noted that BSC is distinct from a collision tumor in that BSC is more of a "mixed tumor" consisting of BCC with areas of SCC differentiation [125] , whereas a collision tumor has distinct BCC and SCC entities that are present in close proximity [126] .…”

“…Recent single case reports appeared in the literature from India [1], Romania [2], UK [3], and USA [4]. Not only was its rarity stressed but also its combining of the histology of basal cell carcinoma and squamous cell carcinoma.…”
